عنوان فارسی بررسی وقوع آواسکولار نکروز سر فمور در بیماران Multiple Sclerosis تحت درمان با اینترفرون بتا
چکیده فارسی مقاله مقدمه: Multiple sclerosis (MS)، بیماری التهابی و دمیلینه کننده‌ی سیستم عصبی مرکزی است و نکروز آواسکولار (Avascular necrosis یا AVN) سر فمور، عارضه‌ای ناشایع در این بیماران می‌باشد. این مطالعه، با هدف بررسی بروز AVN در بیماران تحت درمان با اینترفرون بتا انجام شد. روش‌ها: این مطالعه، پژوهشی مقطعی توصیفی بر روی بیماران MS با دوره‌های عود و بهبود در سال‌های 96-1386 بود. اطلاعات ارزیابی شده، شامل یافته‌های دموگرافیک، زمان شروع MS، زمان تشخیص AVN، دو طرفه یا یک طرفه بودن AVN، مدت زمان، دز و نوع اینترفرون بتا، دز کورتیکو استروئید خوراکی و تعداد دفعات دریافت پالس کورتن بودند. اطلاعات توصیفی به صورت میانگین، انحراف معیار و درصد بیان گردید. یافته‌ها: در مطالعه‌ی حاضر، 6420 بیمار مبتلا به MS با دوره‌های عود و بهبود مورد ارزیابی قرار گرفتند که 4900 بیمار تحت درمان با اینترفرون بتا بودند و وارد این مطالعه شدند. 8 بیمار (16/0 درصد) تحت درمان با اینترفرون بتا، تظاهرات AVN را نشان دادند که میانگین سنی آن‌ها 09/10 ± 25/30 سال بود. 75 درصد بیماران زن و سایرین مرد بودند. میانگین فاصله‌ی زمانی بین بروز MS و بروز تظاهرات AVN، 47/28 ± 12/42 ماه بود. از میان 8 بیمار مبتلا به AVN، 5 بیمار (5/62 درصد) به صورت یک طرفه و 3 بیمار (5/37 درصد) به صورت دو طرفه دچار AVN شدند. 5/87 درصد بیماران مبتلا به AVN تحت درمان با اینترفرون بتا-1a و 5/12 درصد تحت درمان با اینترفرون بتا-1b بودند. نتیجه‌گیری: بر اساس یافته‌های مطالعه‌ی حاضر، این فرضیه مطرح می‌شود که استفاده از اینترفرون بتا ممکن است منجر به بروز AVN در بیماران مبتلا به MS گردد. مکانیسم احتمالی بروز این عارضه، مهار آنژیوژنز، تنگ کردن عروق، تشکیل ترومبوزهای کوچک در آرتریول‌ها و واسکولیت لکوکلاستیک می‌باشد. بنابراین، مطالعات بیشتر در این زمینه توصیه می‌شود.

عنوان انگلیسی Investigating the Occurrence of Femoral Head Avascular Necrosis in Patient with Multiple Sclerosis Treated with Interferon-Beta
چکیده انگلیسی مقاله Background: Multiple sclerosis (MS) is a demyelinating inflammatory disease of central nervous system. Avascular necrosis (AVN) of femoral head is reported as an unusual complaint of multiple sclerosis. In this study, occurrence of AVN due to use of interferon-beta remedies was assessed. Methods: This was a descriptive cross-sectional study on 6420 cases with relapsing-remitting multiple sclerosis from 2007 to 2017. Recorded information were based on the assessment by expert neurologist, and included demographics, time of disease onset, time of the diagnosis of avascular necrosis (AVN), unilateral or bilateral necrosis, duration and dosage and type of beta-interferon, dosage of oral corticosteroid, and pulse of corticosteroid therapy. The data were analyzed, and descriptive information was expressed as mean, standard deviation, and percentage. Findings: In this study, 6420 patients with history of relapsing-remitting multiple sclerosis were evaluated. Of them, 4900 cases were treated with interferon-beta which included in the study. Eight patients presented avascular necrosis of femoral head. Mean age of patients was 30.25 ± 10.09 years. 75% of 4900 cases were treated with interferon-beta were women. Mean duration between multiple sclerosis onset and presentation of avascular necrosis was 42.12 ± 28.47 months. 62.5% of patients had unilateral avascular necrosis. 87.5% of cases were under treatment with interferon-beta-1a and the rest 12.5% treated with interferon-beta-1b. Conclusion: Based on the findings of our study, it is possible to hypothesize that use of interferon-beta may have an effect on occurrence of avascular necrosis among patients with multiple sclerosis. The probable mechanisms could be angiogenesis inhibition, vasoconstriction, and small thrombosis formation in arterioles, as well as leukocytoclastic vasculitis. Further studies are recommended.
