عنوان فارسی بررسی مشارکت مالی خانوارها در هزینه های سلامت بعد از اجرای طرح تحول سلامت
چکیده فارسی مقاله مقدمه: طرح تحول سلامت به عنوان یکی از مهمترین برنامه های وزارت بهداشت با هدف کاهش هزینه های بستری و پرداخت از جیب مردم در سال 1393، آغاز گردید. به منظور بررسی اثرات این برنامه در هزینه های پرداختی مردم، پژوهش حاضر با هدف تعیین مشارکت مالی خانوارها در هزینه های سلامت بعد از اجرای طرح تحول سلامت انجام گردید.مواد و روش کار: این پژوهش مقطعی با هدف کاربردی در نیمه دوم سال 1395و نیمه اول سال 1396در شهرستان زاهدان انجام شد. جامعه پژوهش شامل تمام خانوارهای شهری و روستایی بود که تعداد 816 خانوار به روش خوشه ای و در هر خوشه به صورت تصادفی انتخاب شدند. داده ها به کمک پرسشنامه استاندارد پیمایش سلامت سازمان سلامت جهان و با مراجعه حضوری به منازل خانوارها انجام شد. داده ها با کمک  نرم افزار SPSS v 19 و استفاده از آمار توصیفی و استنباطی از جمله رگرسیون لجستیک تحلیل شد.یافته ها: بر اساس نتایج این مطالعه، سرپرست 760 خانوار مرد بوده و 78 خانوار فاقد بیمه درمانی بودند. پس از اجرای طرح تحول سلامت حدود 12/99 درصد از خانوارها با هزینه های کمرشکن ناشی از هزینه های سلامت مواجه بودند. خانوارهای روستایی بیشتر از خانوارهای شهری با هزینه های کمرشکن مواجه شدند. هزینه های کمرشکن سلامت با استفاده از خدمات دندانپزشکی، دارویی، فیزیوتراپی، سرپایی، وجود فرد نیازمند به مراقبت در خانوار، و وجود فرد بستری در بیمارستان ارتباط معنادار نشان داد.بحث و نتیجه گیری: با وجود اجرای طرح تحول سلامت هنوز بسیاری از خانوارهای شهرستان زاهدان به دلیل استفاده از خدمات سلامت با هزینه های کمرشکن مواجه می شوند و بخشی از آنها به دلیل این هزینه ها به زیر خط فقر کشیده می شوند. نیاز است تا مدیران نظام سلامت نگاه ویژه ای به شهرستان ها و استان های محروم داشته باشند.

عنوان انگلیسی Health expenditures by households after implementation of health transformational plan: a cross-sectional study
چکیده انگلیسی مقاله Objective (s): health transformational plan was executed as one the most important plans by the ministry of health and medical education in order to reduce admission fees. In order to evaluate the effectiveness of this plan, this study was conducted with the goal of determining the health expenditures by households after implementation of this new plan.Methods: This cross-sectional study was conducted in Zahedan, Iran in year 2015 and 2016. The study conducted on 816 families including both rural and urban households. They were selected randomly by clustering method. Data were collected by in-person interviews using the World Health Survey. The Data were analyzed using descriptive and inferential statistics.Results: In 93.1% of instances the head of household were men (n = 760) and 78 families did not have any health insurance. After implementation of health transformational plan, 12.99 percent of families were still faced with catastrophic health expenditures. However, the rural families were facing more catastrophic expenditures compared to families living in urban areas. This catastrophic expenditures have a meaningful relation with many variables including dental care (OR: 4.99), drug fees (OR: 2.927), physiotherapy (OR: 2.927), outpatient expenditures (OR: 96. 639), having a family member who needs to be taken care of at home (OR: 3.186), and a family member who needs to be taken care of at a hospital (OR: 1.866).Conclusion: The results showed that a relatively large proportion of people in families in Zahedan still are facing catastrophic health related expenditures and health transformational plan has not been able to reach its goals in this city.
