عنوان انگلیسی Effectiveness of Methylene Blue in Prevention of Stifle Joint Arthrofibrosis in Rabbit Models
چکیده انگلیسی مقاله Introduction: Prevention of arthrofibrosis is a major challenge in orthopedic surgery and can be successfully alleviated in its early stages. Many studies suggest methylene blue (MB) in prevention of post-operation adhesions as an accessible and effective agent. The purpose of this study was to evaluate the efficacy of MB in prevention of arthrofibrosis. Methods: The anterior cruciate ligaments (ACL) of the left stifle joint of 18 mature New Zealand white female rabbits were cut under aseptic surgery. The animals were then divided into three groups based on the treatment. Saline, 1% MB, and 2% MB solutions were injected into the synovial spaces of groups one, two, and three, respectively. The range of motion was measured post-operation in operated stifle joints every week. The animals were euthanized after four weeks and the whole joints were dissected for histopathology. Results: Histopathology showed fibrotic connective tissue as a sign of fibrosis in all groups. The most fibrosis rate, inflammatory rate, tissue disarrangement, fibroblastic cellularity and granulation tissue formation were observed in the 1% MB group. The integrity of articular cartilage in the 2% MB group was lower than the other groups. Bone degeneration was similar in both MB groups, but more than the control saline group. The ROM was reduced during the 1st week, but increased in the 2nd and 3rd weeks and decreased again in the 4th week. Conclusion: Methylene blue is not a valuable medium for prevention or treatment of arthofibrosis and the subsequent osteoarthritis. Contrary to the previous reports, the fibrosis was lower in the high dose compared to the low dose MB group. The alterations in ROM are related to pain and stiffness restricted movement in the operated joints. This is the first report on the adverse effect of MB on the synovial components.
