عنوان فارسی گزارش یک مورد نادر بثورات منتشر ناشی از مسمومیت با متوترکسات
چکیده فارسی مقاله مقدمه : متو ترکسات  بعنوان یک داروی آنتی‌ متابولیت‌ ، هر چند دارای عوارض متعددی است ، ولی بروز پتشی های منتشر جلدی ( بثورات ماکولو پاپولر) از عوارض بسیار نادرآن  حتی در دوزهای پائین  میباشد. معرفی مورد: بیمار یک زن 77 ساله به دلیل اجرای نادرست دستور نحوه مصرف داروی متوتروکسات ، با راش های پوستی منتشر علیرغم مصرف داروهای متعدد برای درمان بیماری مزمن روماتیسمی از قبل موجود  به بخش اورژانس بیمارستان آورده شده بود ،  بیمار با شک به مسمومیت با متوتروکسات ، پس از بررسی های لازم و نیز اقدامات درمانی اولیه ،تحت درمان با فاکتور محرک کلونی ماکروفاژ- گرانولوسیت قرار گرفت و به طور شگفت انگیزی تمام عوارض بالینی وآزمایشگاهی ایجاد شده بهبود یافت. نتیجه گیری : در زمان مصرف متوتروکسات  توسط بیمار حتی با تجویز پزشک متخصص ،با  بروز هرگونه نشانه های پوستی نظیر راش های ماکولوپاپولر منتشر بدون  خارش  باید به فکر مسمومیت با متوتروکسات  بود .

چکیده انگلیسی مقاله  Background:  Methotrexate is an anti-metabolite Drug and one of the folic acid analog   that it can play an important and functional role in the treatment of many malignancies and inflammatory diseases. However, the development  of petechia ( Maculopopular  Rash) is a very rare complication .   Case presentation: The patient is a 77-year-old woman because of a malfunctioning order of taking methotrexate,with severe and all over skin rashes, despite the use of multiple drugs for the treatment of chronic pre-existing rheumatic Disease, was brought to The emergency department  of the hospital. That the patient mistakenly, while he was directly responsible for taking the medication, consumed 3 times daily for 4 days each day, In the examination of oral and throat mucosa there were mucositis symptoms, There were numerous petetial lesions in the upper anterior chest region, above the arms and shoulders that lacked prominence and erythema around them, Which lacked prominence and erythema around it, Skin lesions without itching, In the funduscopy, the bottom of the eyes was normal, the conjunctival eyes were prurient, Other patient examinations were normal. . In the trials before treatment, the patient had high levels of creatinine, and white blood cells and Platelet levels were very low.other tests is normal. This suggests that methotrexate can cause side effects even at low doses. In the follow up all abnormal laboratory tests were normal reporting such cases That It can greatly help physicians. Conclusion: At the time of taking methotrexate by the patient, even with the appointment of a specialist physician, with any skin signs such as maculopapular rash without itching، You should consider poisoning with methotrexate, and think appropriately about it.
