عنوان انگلیسی Triceps-Sparing Posterior Approach for Supra-Condylar Fractures in Children
چکیده انگلیسی مقاله Background: The most common paediatric elbow fracture is supracondylar humeral fracture which accounts for 60% of elbow fractures in children. The aim of our study was to evaluate the results of open reduction and internal fixation (ORIF) of type III supracondylar humeral fractures using a Triceps sparing posterior approach. Methods: From June 2007 to June 2014, ninety eight patients were evaluated. Results: The mean age was 6.4 years. The ratio of boys to girls and right to left hand was 2.06 and 2.26 respectively. 82% of fractures happened in dominant hand which was right. The patients underwent surgery within 50.16 hours after injury averagely. Anatomic reduction and internal fixation was performed under direct vision with no need to image intensifier. Mean time of follow up was 14.3 months. All fractures healed clinically and radiologically. Maximum lack of extension was 15° and its mean was 3.5°. Mean Final Bauman angle difference with healthy elbow was 2.4°.Complications were 19.3% including: pin tract infections (7%), pin loosening (3%), heterotopic ossification (4%), and wound dehiscence (1%). There were 4 cases (4%) of anterior interosseous nerve Palsy, two of which happened before and the other two, after surgery. All were resolved within 3 to 10 weeks spontaneously. Conclusion: Using this approach helps to preserve extensor mechanism and ulnar nerve intact and to have an acceptable skin scar esthetically along with satisfactory postoperative function.
