چکیده فارسی مقاله چکیده زمینه و هدف: امروزه علی‌رغم پیشرفت‌های متعدد علمی و تکنولوژیک در تشخیص و درمان بیماری‌ها روند شکایت از پزشکان و کادر معالج رو به افزایش است. افزایش جمعیت، افزایش تعداد پزشکان، افزایش آگاهی جامعه و استفاده از ابزارهای تشخیصی و درمانی پیچیده‌تر از علل مهم این روند رو به رشد هستند و در این میان کاهش روابط سنتی میان پزشک و بیمار بیشتر به این موضوع دامن می‌زند. هدف از این مطالعه بررسی علت و انگیزه های شکایت، میزان و نوع قصور انجام شده و در آخر ارایه راه حل برای کاهش قصور پزشکان می‌باشد. روش بررسی: این مطالعه توصیفی تحلیلی به صورت گذشته‌نگر با بررسی پرونده‌های شکایت از قصور پزشکی در رشته تخصصی ارتوپدی طی سال‌های 1377 تا 1382 در اداره‌کل پزشکی قانونی استان تهران انجام شده است. یافته ها: طی این مدت از بین 965 پرونده شکایت از قصور پزشکی 196 مورد (31/20%) به این رشته اختصاص داشته و از 15 مورد در سال 1377 به 39 مورد در سال 1382 افزایش یافته است. در 72/60% پرونده‌ها رأی برائت و در 28/39% رأی قصور صادر گردیده است. 59/54% پرونده‌ها به مراکز خصوصی و 86/17% به مراکز دولتی غیر دانشگاهی و 94/21% به مراکز دانشگاهی و 61/5% به مراکز خیریه مربوط بودند؛ ولی میزان قصور در مراکز خصوصی 34/52% ، در مراکز دولتی غیر دانشگاهی 86/22% و در مراکز دانشگاهی 93/20% و در مراکز خیریه 36/36% بود. 09/9% از موارد قصور به دنبال فوت بیمار و مابقی به دنبال نقص عضو و کاهش کارآیی اتفاق افتاده بود. نتیجه‌گیری: بهبود مناسبات پزشک و بیمار، رعایت اخلاق پزشکی، کاهش روابط مالی میان پزشک و بیمار، افزایش دانش و مهارت‌های علمی و فنی پزشکان، توجیه مناسب بیماران قبل از انجام فرآیند‌های تشخیصی یا درمانی و ارتقای سطح فنی و تجهیزاتی مراکز درمانی در کاهش قصور و شکایت تأثیر بسزایی دارد.

عنوان انگلیسی Assessment of records of complaints from medical malpractice in the field of orthopedic, in the coroner’s Office of Forensic Medicine, province of Tehran, during 1988 to 2003
چکیده انگلیسی مقاله Abstract Background and Aim: In spite of the numerous advancements in diagnosis and treatment of the disease, today, lodging files against physicians and medical assistants is increasing. Population growth, social awareness growth, and growing number of physicians, as well as implementation of complex and modern devices in diagnosis and treatment of the diseases are among the reason for such complains. Moreover, the lessening of traditional relationship between patient and physician accelerates this phenomenon. The aim of our study was to asses etiologies and motivations of complains, rate and type of malpractice and at last find a solution for decreasing physicians’ Malpractice. Materials and Methods: In a retrospective descriptive-Analytic study we evaluated the records of medical malpractice in the field of orthopedic during 1998 to 2003, in the coroner's office of Forensic Medicine, province of Tehran. Findings: During this time among 965 cases of medical malpractice, 196 (20.31%) have been related to this field. This process has increased from 15 cases in 1998 to 39 cases in 2003. In 60.72% of the cases acquittal verdict, and in 39.28% of the cases malpractice verdict have been issued. Of the total number of the cases 54.59% of them have been related to private centers, 17.86% related to non academic governmental centers, and 21.94% were related to university centers and 5.61% related to charity centers. However, malpractice rate in these centers has been 52.34%, 22.86%, 20.93% and 36.36% respectively. 9.09% of the malpractice cases resulted in the death of the patient, the rest resulted in physical handicap, and deficiency. Conclusion: Improvement in the patient - physician relations, implementation of medical ethics, decrease in related financial affairs, improvement of professional skills and knowledge of the physicians, adequate briefing of the patients before diagnostic and therapeutic procedures, and accommodation of therapeutic centers with the advanced technical equipments has significant effect on the lessening of ignorance and complaints
