عنوان فارسی تبیین وضعیت مدیریت تخت‌های مراقبت ویژه: یک مطالعه کیفی
چکیده فارسی مقاله مقدمه: دسترسی به تخت‌های مراقبت ویژه در کشورهای جهان متفاوت است؛ اما تقاضا برای این تخت‌ها به دلیل رشد سالمندی جمعیت، درنتیجه بقای طولانی‌تر بیماران مزمن در همه کشورها رو به افزایش است. این مطالعه به تبیین وضعیت مدیریت تخت‌های مراقبت ویژه در بیمارستان‌های آموزشی منتخب تابعه دانشگاه علوم پزشکی شهید بهشتی پرداخت. روش‌: این مطالعه با رویکرد کیفی و روش تحلیل محتوای قراردادی در سال 1395 انجام شد. 25 مشارکت‌کننده به‌صورت نمونه‌گیری مبتنی بر هدف و با حداکثر تنوع انتخاب شدند؛ و با سؤالات باز مورد مصاحبه نیمه ساختاریافته قرار گرفتند. مکان مصاحبه، محل کار افراد بود و زمان مصاحبه‌ها بین 30 تا 60 دقیقه به طول انجامید. داده‌ها با استفاده از رویکرد تحلیل محتوای قراردادی تجزیه‌وتحلیل گردید. یافته‌ها: با تجزیه‌وتحلیل مصاحبه‌ها، 4 موضوع اصلی (ظرفیت‌های انسانی مرتبط با مدیریت تخت، الزامات سازمانی و مدیریتی مرتبط، چگونگی پذیرش و ترخیص بیماران واجد اولویت در ICU و راهکارهای ارتقاء و بهبود وضعیت مدیریت تخت‌های ICU) و 12 موضوع فرعی در خصوص وضعیت مدیریت تخت‌های مراقبت ویژه استخراج شد. نتیجه‌گیری: بر اساس یافته‌های این مطالعه، مدیریت تخت در بخش مراقبت ویژه کشور ما با چالش‌های زیادی به‌ویژه در حیطه‌های ظرفیت‌های انسانی مرتبط، نحوه اولویت‌گذاری پذیرش و ترخیص و نظارت و کنترل بخش‌های ICU روبرو است؛ که ضرورت توجه بیشتر در این موارد به‌شدت به چشم می‌خورد و رفع این چالش‌ها از طریق آموزش تعداد بیشتری از متخصصین مراقبت ویژه، رعایت معیارهای بین‌المللی پذیرش و ترخیص و استفاده از دستگاه‌های نظارتی و کنترلی مناسب امکان‌پذیر است.

عنوان انگلیسی Explaining Management Status of Intensive Care Units Beds: A Qualitative Study
چکیده انگلیسی مقاله Introduction: Access to intensive care unit (ICU) beds varies in different countries. Due to the population growth, aging, and longer survival of chronic patients, demand for these beds is increasing all over the world. The present study was performed to assess the management status of ICU beds in selected teaching hospitals affiliated to Shahid Beheshti University of Medical Sciences. Method: This qualitative study was conducted using conventional content analysis method in 2016. In this study, 25 participants were selected by purposive sampling and with maximum variation, and were interviewed by semi-structured interviews with open-ended questions. The interviews were performed in the participants' workplace and lasted between 30 and 60 min. Data were analyzed using conventional content analysis approach. Results: After data analysis, 4 main themes including human capacities related to bed management, the relevant organizational and managerial requirements, ICU prioritized patients' admission and discharge manner, and strategies for promotion and improvement of ICU bed management status and 12 subthemes related to the management status of ICU beds were extracted. Conclusion: According to the results, in our country, bed management in the intensive care units faces many challenges, especially in the areas related to human capacities, ICU admission and discharge prioritization method, and ICUs' monitoring and control, which need to be paid more attention. These challenges can be addressed through training more ICU specialists, observing international admission and discharge criteria, and using appropriate monitoring and control systems.
