Spontaneous ovarian hyperstimulation syndrome in second pregnancy of a healthy pregnant woman

Background: Spontaneous ovarian hyperstimulation syndrome (OHSS) is an uncommon type of OHSS which is characterized by gastrointestinal symptoms and complications of accommodation of body fluids in third spaces in the absence of medical ovarian stimulations. This syndrome is mostly seen in multiple or molar pregnancies and with an underlying medical conditions such as hypothyroidism. Treatment of spontaneous OHSS depend on the patients clinical condition. Appropriate management will warrant a successful pregnancy. In present report, Case report: In present paper, we have reported an 8 weeks pregnant female referred to gynecology and obstetrics clinic because of gradual abdominal distension, abdominal pain and nausea started a month eariler. The young woman didn't have any history of specific predisposing factor of OHSS such as thyroid gland dysfunction or multiple pregnancy. Sonography of the abdomen showed enlarged ovaries with prominant follecules as well as free fluid in entire abdomen. Elevated level of HCG (312850mlU/ml) and carcinoemberionic antigen (106U/ml) were remarkable labratory findings. We could successfully control the patient syndrome by conservative management and the size of ovaries returned back to normal by 17th weeks of gestation. On sonographic follow ups till delivery the patient was symptom free and gave birth to a healthy infant. Conclusion: While spontaneous OHSS is a rare and life threatening condition, conservative management including albumin and anticoagulant administration as well as paracentesis of body third space fluids will be a promising approach in stable patients.
