عنوان فارسی ملاحظات بالینی در تشخیص افتراقی کودکان دارای اختلال طیف نوروپاتی شنوایی از کم‌شنوایی حسی- عصبی: گزارش مورد
چکیده فارسی مقاله مقدمه: اختلال طیف نوروپاتی شنوایی (Auditory neuropathy spectrum disorder یا ANSD)، نوعی آسیب شنوایی متمایز از کم‌شنوایی حسی- عصبی در دستگاه شنوایی محیطی است. هدف از انجام پژوهش حاضر، تأکید بر اهمیت بهره‌گیری از رویکرد مجموعه‌ی آزمون‌ها و نقش خانواده در تشخیص افتراقی ANSD از کم‌شنوایی حسی- عصبی معمول در قالب گزارش دو کودک با آسیب شنوایی بود. گزارش مورد: کودک اول، پسر بچه‌ی 19 ماهه‌ی دچار فلج مغزی با سابقه‌ی کرنیکتروس (Kernicterus) و دو بار تعویض خون بود که ابتدا با استفاده از آزمون‌های الکتروفیزیولوژیک شنوایی، کم‌شنوایی حسی- عصبی عمیق دو طرفه در وی تشخیص داده شده بود. با توجه به عدم پذیرش سمعک و تردید والدین در مورد واکنش‌های نامتناسب کودک، در ارزیابی مجدد با رویکرد مجموعه‌ی آزمون‌ها، ANSD در او عنوان شد. کودک دوم، دختر بچه‌ی 6 ساله‌ی بدون سابقه‌ی خاص پزشکی بود که در غربالگری بدو ورود به دبستان ارجاع شد. در جلسه‌ی اول، ارزیابی شنوایی با استفاده از ادیومتری و ایمیتانس اکوستیک، کم‌شنوایی حسی- عصبی ملایم تا متوسط دو طرفه با افت بیشتر در فرکانس‌های پایین را گزارش کرد. به دلیل امتیاز کم بازشناسی کلمه و عدم پذیرش والدین، آزمون‌های الکتروفیزیولوژیک انجام و ANSD تأیید گردید. نتیجه‌گیری: تاریخچه‌گیری دقیق، ارزیابی شنوایی کودکان دارای کم‌شنوایی حسی- عصبی با پروتکل‌های ANSD که ترکیب متناسبی از آزمون‌های شنوایی رفتاری و الکتروفیزیولوژیک است، همراه با افزایش آگاهی خانواده، عوامل مهمی در تشخیص افتراقی ANSD و مداخله‌ی مناسب می‌باشد.

عنوان انگلیسی Clinical Considerations in the Differential Diagnosis of Children with Auditory Neuropathy Spectrum Disorder from Sensorineural Hearing Loss; Case Report
چکیده انگلیسی مقاله Background: Auditory neuropathy spectrum disorder (ANSD) is a type of hearing impairment distinct from sensorineural hearing loss in the peripheral auditory system. The purpose of this study was to emphasize the importance of using the test battery approach and the role of the family in the differential diagnosis of ANSD from common sensorineural hearing loss in the form of reports of two children with hearing impairment. Case Report: The first child, a 19-month-old boy with cerebral palsy with a history of kernicterus and twice of blood exchange. He was first diagnosed with bilateral profound sensorineural hearing loss using electrophysiological tests. Due to the rejection of the hearing aid and the parent' doubts about the disappropriate reactions of the child, in re-evaluation via test battery approach, ANSD was obtained. The second child was a 6-year-old girl with no specific medical history in school entrance screening. In the first session of hearing evaluation using audiometry and acoustic immittance, bilateral mild to moderate sensorineural hearing loss was achieved with more loss at low frequencies. Due to the low word recognition score and parental denial, electrophysiological tests were performed, and ANSD was confirmed. Conclusion: Accurate case history, hearing assessment of children with sensorineural hearing loss with ANSD protocols, which is an appropriate combination of behavioral and electrophysiological hearing tests, as well as increased family awareness, are important factors in the differential diagnosis of ANSD and selection of appropriate intervention.
