عنوان فارسی نقش سیستم کامپیوتری ثبت دستورات پزشکی در کاهش خطاهای دارویی
چکیده فارسی مقاله مقدمه: امنیت بیمار یکی از مهمترین ابعاد مراقبت بهداشتی است اما خطاهای پزشکی و بخصوص خطاهای دارویی موجب نا امن شدن سیستم مراقبت بهداشتی شده است. تکنولوژی اطلاعات بخصوص سیستم کامپیوتری ثبت دستورات پزشکی رویکردی مناسب برای کاهش خطاهای دارویی است؛ لذا هدف این مطالعه بررسی علل بروز خطاهای دارویی و تعیین قابلیت های لازم برای طراحی سیستم کامپیوتری ثبت دستورات پزشکی در ایران می‌باشد. مروری بر مطالعات: علت اصلی بسیاری از خطاهای پزشکی دسترسی ناکافی به اطلاعات در زمان تصمیم گیری و ارتباطات ضعیف بین اعضاء تیم مراقبت است که در سیستم های دستی بیشتر به چشم می خورد. دلیل اصلی خطاهای دارویی فقدان اطلاعات کافی، نبود کنترل و کم توجهی به اطلاعات مربوط به بیمار یا دارو است؛ بنابراین وجود سیستم ثبت و گزارش دهی داده ها از مهمترین عوامل کاهش خطاها محسوب می‌گردد ولی سیستم دستی کفایت لازم برای پیشگیری از این خطاها را ندارد. از طرفی ثابت شده است که سیستم کامپیوتری ثبت دستورات پزشکی امکان کاهش خطاهای دارویی، پیامدها و هزینه های آن را به میزان زیادی داراست. نتیجه گیری: سیستم کامپیوتری ثبت دستورات پزشکی رویکردی مناسب برای دسترسی بهنگام به اطلاعات بیمار و دارو در زمان تجویز و کاهش خطاهای دارویی است. برای طراحی این سیستم در ایران توجه به فراهم کردن قابلیت هایی چون آگاه کردن پزشکان سوابق دارویی بیمار، ثبت الزامی اطلاعات، یادآوری و هشداردهی موارد مهم و تاثیر گذار بر درمان، سیستم پشتیبان تصمیم، ارتباط با سایر سیستم های اطلاعاتی، ارایه اطلاعات آموزشی در مورد داروها، بکارگیری تکنولوژی اینترنت و طراحی پروتکل های درمانی گوناگون از اهمیت زیادی برخوردار است.

چکیده انگلیسی مقاله Introduction Literature review: Untimely access to information at time of decision making and weak communication among health care providers are the main causes of medical errors. In addition, unavailability and lack of attention to information related to patients and drugs are the most common reasons for medication errors. So data reporting systems can support patients' safety. Manual systems, however, are not adequate to this end. In addition, CPOE has a special role at time of drug prescription to decrease medication errors. Conclusion: Using CPOE is a suitable approach to have access to timely information and to improve the relationships among specialists, and therefore to reduce errors. The results of our study show the necessary capabilities to design CPOE for Iranian hospitals. Patients' drug profiles and informing physicians about using drugs and patients' inactive drugs, obligatory documentation, warning ability and reminding important and effective points on treatment, creating facilities to support decisions, relationship between CPOE to other information systems, presenting educational information about prescribed and replaced drugs, using internet and treatment protocols, are some such capabilities that all affect physicians' prescription behaviors and finally lead to improvement in drug prescription process and reduction of medication errors. : Patient safety is one of the most important aspects of health care. Medical errors and specifically medication errors, however, make health care environment dangerous. Computerized Physician Order Entry (CPOE) has potentials to reduce these errors. So this study aimed to analyze the patients' safety condition and to determine the CPOE capabilities to decrease medication errors and create a safe health care environment for Iranian patients.
