عنوان فارسی تعیین موانع مشارکت در درمان بیماران مبتلا به سرطان پستان در ایران
چکیده فارسی مقاله  مقدمه: مشارکت بیمار در تصمیم گیری درمان در بسیاری از کشورها و سیستم های بهداشتی سراسر جهان به یک نیاز سیاسی تبدیل شده است. در سطح جهان نحوه برخورد با بیماران مبتلا به سرطان رویکرد متفاوتی پیدا کرده است. با توجه به این که سرطان سلامت جسمی، روحی و اجتماعی فرد را تحت تاثیر قرار می دهد، مراقبت بیمار محور می تواند یکی از رویکردهای مناسب در بیماران مبتلا به سرطان باشد(1). هدف از انجام این مطالعه تعیین موانع و عوامل تاثیر گزار بر میزان مشارکت بیماران در درمان است. روش بررسی: مطالعه حاضر، یک مطالعه کیفی است که با استفاده از مصاحبه نیمه ساختارمند با همکاری 15 نفر از پزشکان جراح پستان و 300 نفر از بیماران مبتلا به سرطان پستان که به مطب و درمانگاه های پزشکان در دانشگاه علوم پزشکی شیراز، تبریز،گیلان و کرمان مراجعه کرده بودند، انجام شد. نمونه گیری بیماران با روش تصادفی خوشه ای به ازای هر پزشک 20 بیمار صورت گرفت. تایید روایی و پایایی سئوالات با استفاده از نظرات 15 نفر از خبرگان انجام شد. در نهایت با استفاده از مدل پارادایمی به شیوه گرند تئوری کدگذاری و دسته بندی داده ها انجام گردید. یافته ها:. نتیجه پژوهش نشان داد که ترس و اضطراب بیماران مهمترین عامل در عدم همکاری و مشارکت ضعیف بیماران در درمان می باشد. عدم آگاهی و اطلاعات ناکافی بیماران دومین مانع در مشارکت ضعیف در تصمیم گیری درمان بود. همچنین یافته ها حاکی از آن بود که هزینه های درمانی با وجود بروز مشکلات فراوان برای بیماران تاثیری در ادامه روند درمان و نوع درمان انتخابی نداشتند. بحث و نتیجه گیری: با توجه به تاثیر اضطراب و سطح آگاهی و تحصیلات بیماران به عنوان مهمترین موانع مشارکت بیمار در درمان پیشنهاد می گردد، با استفاده از راهکارهای مشاوره و روان درمانی هم راستا با درمان بیماری و توانمند سازی بیماران با اطلاع رسانی عمومی و برگزاری کلاس های آموزشی در مراکز بهداشتی گامی موثر در افزایش مشارکت بیماران در درمان برداشته شود.

عنوان انگلیسی Determination Barriers to Participation in the Treatment of Breast Cancer Patients in Iran
چکیده انگلیسی مقاله  Introduction: Patient participation in treatment decisions has become a political necessity in many countries and health systems around the world. Globally, the treatment of cancer patients has taken a different approach. Given that cancer affects a person's physical, mental and social health, patient-centered care can be one of the appropriate approaches in cancer patients(1). The aim of this study was to determine the barriers and factors affecting patients' participation in treatment. Methods: The present study is a qualitative study using semi-structured interviews with 15 breast surgeons and 300 breast cancer patients who went to doctors' offices and clinics in Shiraz University of Medical Sciences, Tabriz, Guilan. And Kerman had come, it was done. Patients were sampled by cluster random sampling for 20 patients per physician. Confirmation of validity and reliability of the questions was done using the opinions of 15 experts. Finally, using paradigm modeling, Grand Theory was used to encode and classify the data. Findings: The results showed that patients 'fear and anxiety is the most important factor in patients' lack of cooperation and poor participation in treatment. Lack of knowledge and insufficient information of patients was the second obstacle to poor participation in treatment decisions. The findings also showed that the cost of treatment, despite the many problems for patients, had no effect on the continuation of the treatment process and the type of treatment selected. Discussion and Conclusion: Considering the effect of anxiety and the level of knowledge and education of patients as the most important barriers to patient participation in treatment is proposed, using counseling and psychotherapy strategies in line with the treatment of the disease and empowering patients with public information and Holding training classes in health centers is an effective step in increasing patient participation in treatment
