چکیده فارسی مقاله Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal" mso-tstyle-rowband-size:0 mso-tstyle-colband-size:0 mso-style-noshow:yes mso-style-priority:99 mso-style-qformat:yes mso-style-parent:"" mso-padding-alt:0cm 5.4pt 0cm 5.4pt mso-para-margin:0cm mso-para-margin-bottom:.0001pt mso-pagination:widow-orphan font-size:11.0pt font-family:"Calibri","sans-serif" mso-ascii-font-family:Calibri mso-ascii-theme-font:minor-latin mso-fareast-font-family:"Times New Roman" mso-fareast-theme-font:minor-fareast mso-hansi-font-family:Calibri mso-hansi-theme-font:minor-latin mso-bidi-font-family:Arial mso-bidi-theme-font:minor-bidi} زمینه و هدف: یکی از مشکلات جراحی استابولوم دسترسی و دید (Exposure) مناسب است. استئوتومی تروکانتریک Flip یکی از روش‌های جراحی برای دست‌یابی دیواره خلفی فوقانی می‌باشد. هرچند به علت ترس از بعضی عوارض برخی جراحان از انجام آن پرهیز می‌کنند. هدف این مطالعه بررسی پیامد انجام درمان جراحی با استفاده از استئوتومی تروکانتریک Flip در بیماران با شکستگی استابولوم مراجعه‌کننده به بیمارستان امام‌خمینی (ره) بین سال‌های 1384 تا 1387می‌باشد. روش بررسی: تعداد 14 بیمار به این مطالعه وارد شدند. بیماران حداقل تا یک سال پس از عمل پی‌گیری شدند. اطلاعات دموگرافیک، نتیجه رادیوگرافی، میزان درد بر اساس Visual Analogue Scale (VAS)، نمره Harris Hip Score (HHS)، قدرت عضلات ابداکتور و عوارض ایجاد شده ثبت گردید. یافته‌ها: میانگین HHS بیماران 5/82 با محدوده 95-55 بود. تعداد سه نفر دچار استخوان‌سازی نابه‌جا شدند. هیچ موردی از عفونت بعد از عمل و نان یونیون قطعه استئوتومی شده گزارش نشد. تنها در دو نفر جابه‌جایی قطعه استئوتومی شده مشاهده شد. میزان ریداکشن در 10 نفر آناتومیک بود. تنها در یک نفر قدرت عضلات ابداکتور هیپ در حد سه پنجم بود. میانگین شدت درد در بیماران تحت مطالعه 4/3 با محدوده 7-1 بود. هیچ موردی از استئونکروز سر فمور گزارش نشد. وضعیت مفصل هیپ با توجه به HHS در چهار و شش بیمار به ترتیب عالی و خوب بود. در سه بیمار متوسط و تنها یک بیمار در طبقه ضعیف قرار گرفت. نتیجه‌گیری: به نظر می‌رسد استئوتومی تروکانتریک Flip عوارض کم‌تری نسبت به استئوتومی‌های رایج دارد و استفاده از آن در درمان شکستگی‌های استابولوم توصیه می‌شود.

Trochanteric flip osteotomy is one of the surgical methods for superoposterior and posterior acetabulum exposure. However, due to possible complications some surgeons prefer to avoid this procedure. This study was undertaken to determine the outcome of surgical treatment of acetabular fracture using trochanteric flip osteotomy. Methods : In this prospective cohort study, 14 patients with acetabular fracture who had been admitted in Imam Khomeini Hospital in Tehran, Iran, during 2003-2006 underwent trochanteric flip osteotomy. The patients were followed for at least one year post-surgically. Demographics, radiologic findings, intensity of pain using visual analogue scale (VAS), Harris hip score (HHS), force of hip abductors and complications were noted. Data analysis was performed using SPSS ver. 13.Results : The mean HHS was 82.5 (55-95). Heterotopic ossification was observed in three patients. There were no cases of postoperative infection or non:::::union:::::. Only two patients showed displacement of osteotomized fragments. Reduction was anatomic in 10 patients. In one patient, the force of hip abductors was three-fifth. The mean hip pain was 3.4 based on VAS. There were no cases of femoral head osteonecrosis. With respect to HHS, the final hip status was excellent and good in four and six patients, respectively. Three patients had fair and only one patient had poor condition.Conclusion: It seems that trochanteric flip osteotomy has much fewer complications in comparison to other methods justifying its use in such cases.
