عنوان فارسی اثر آموزش و ارائه پسخوراند کتبی بر کمیت و کیفیت مستند سازی پرونده بیماران در یک بیمارستان آموزشی عمومی: یک مطالعه اقدام پژوهی
چکیده فارسی مقاله مقدمه و اهدف: در مطالعات متعدد انجام گرفته در ایران نشان داده شده است که کیفیت مستندسازی پرونده های پزشکی توسط پزشکان و کادر درمانی در ایران  چندان رضایت بخش نیست.  با توجه به  اهمیت مستندسازی صحیح پرونده های پزشکی  در بیمارستانهای آموزشی بعنوان سندی معتبر از فعالیتهای آموزشی و درمانی صورت گرفته در اعتباربخشی های آموزشی، مطالعه حاضر با هدف بررسی اثر آموزش بر کمیت و کیفیت مستندسازی پرونده های بیماران طراحی و اجرا شد.
روش کار:  مطالعه حاضر از نظر طراحی یک مطالعه اقدام پژوهی بود. در این مطالعه ابتدا وضعیت ثبت پرونده های پزشکی توسط دستیاران، کارورزان و کارآموزان در400 پرونده انتخاب شده به صورت تصادفی مورد ارزیابی قرار گرفت. سپس براساس یک پروتکل مداخله ای، برنامه های آموزشی برای این افراد برگزار و به صورت ماهیانه موارد نقص در پرونده ها هم در قالب بازخورد کتبی به مدیران گروه های آموزشی ارائه گردید. در نهایت وضعیت ثبت داده ها با استفاده از نرم افزار SPSS V.20  و آزمون مک نمار آنالیز شد.
نتایج: بر اساس مقایسه سنجه های مورد بررسی قبل و بعد از انجام مداخله، درصد ثبت شرح حال، درصد On service note، درصد Off service note و درصد Progressive note (PN)روزانه در هر سه گروه کار آموزان پزشکی، کارورزان و دستیاران بهبود معنی داری داشت (05/0> P).  علاوه بر اینها، مداخله انجام شده باعث بهبود معنی داری در سایر شاخص های مورد بررسی از جمله اصول صحیح تشخیص نویسی در پرونده، ثبت ساعت و تاریخ، ثبت دستورات دارویی، تکمیل فرم آموزش بیمار، اخذ رضایت آگاهانه از بیمار در موارد نیاز، ثبت صحیح درخواست و انجام مشاوره های پزشکی و تهیه خلاصه پرونده استاندارد شده بود(05/0> P).
نتیجه گیری:  بر اساس یافته های این مطالعه، آموزش مستمر و ارائه بازخورد کتبی منظم در افزایش کمیت و کیفیت ثبت موارد لازم در پرونده های پزشکی بیماران توسط دستیاران، کارورزان و کارآموزان موثر است.

عنوان انگلیسی Effect of Education and Written Feedback on the Quantity and Quality of Documenting Patients' Records in a General Teaching Hospital: An Action Research Study
چکیده انگلیسی مقاله Background and Aim: Numerous studies conducted in Iran have shown that the quality of documentation of medical records by physicians and medical staff is not very satisfactory. Considering the importance of proper documentation of medical records in teaching hospitals as a valid document of educational and medical activities in educational accreditation, the present study was designed and conducted to investigate the effect of training on the quantity and quality of documentation of patients' records.

Materials and Methods: The present study was an action research study in terms of design. In this study, the status of medical records was first evaluated by assistants, interns and trainees in 400 randomly selected files. Then, based on an intervention protocol, training programs were held for these people and monthly defects in the files were provided to the department heads in the form of written feedback. Finally, the data analyses conducted in SPSS V.22 software using McNemar's test.

Results Based on the comparison of the studied criteria before and after the intervention, the percentage of history recording, the percentage of on-service note, the percentage of off-service notes, and the percentage of daily progressive note (PN) in all three groups of medical students including externs, interns and residents improved significantly. (P <0.05). In addition, the intervention significantly improved the other indicators, including the principles of correct diagnosis in the file, recording the time and date, recording medication orders, completing the patient education form, obtaining informed consent from the patient in case of need, correctly recording of requested and performed medical consultations and preparation of standardized file summary (P <0.05).

Conclusions: Based on the findings, continuous training and providing regular written feedback is effective in increasing the quantity and quality of registration of necessary cases in patients' medical records by assistants, interns and trainees.
