چکیده فارسی مقاله Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal"; mso-tstyle-rowband-size:0; mso-tstyle-colband-size:0; mso-style-noshow:yes; mso-style-priority:99; mso-style-qformat:yes; mso-style-parent:""; mso-padding-alt:0cm 5.4pt 0cm 5.4pt; mso-para-margin:0cm; mso-para-margin-bottom:.0001pt; mso-pagination:widow-orphan; font-size:11.0pt; font-family:"Calibri","sans-serif"; mso-ascii-font-family:Calibri; mso-ascii-theme-font:minor-latin; mso-fareast-font-family:"Times New Roman"; mso-fareast-theme-font:minor-fareast; mso-hansi-font-family:Calibri; mso-hansi-theme-font:minor-latin; mso-bidi-font-family:Arial; mso-bidi-theme-font:minor-bidi;} مقدمه ارتفاع مچ دست، یک نسبت رادیولوژیک با اهمیت بالینی در بیماری های روماتولوژیک و کینباک است که به روش های مختلفی اندازه گیری می شود. هدف این مطالعه، بررسی دو روش مشهور اندازه گیری این نسبت از نظر اختلاف بین اندازه گیر و اندازه گیران بود. روش ها از 10 رادیوگرافی با کیفیت مناسب، عکس گرفته شد و دو نفر ارتوپد با تجربه، در دو نوبت متفاوت، طول متاکارپ سوم، بیشترین طول کاپیتیت و فاصله ی متاکارپ سوم تا سطح مفصلی دیستال رادیوس را اندازه گرفتند. بقیه ی محاسبات توسط رایانه انجام شد. یافته ها پایایی درون اندازه گیر نسبت Youm در حد بالا و پایایی درون اندازه گیر نسبت Nattrass خوب محاسبه شد، ولی در مورد پایایی بین اندازه گیر، دقت هر دو نسبت در حد ضعیف بود. پایایی درون و بین اندازه گیر در مورد بازگویی فروپاشی در مچ دست، برای نسبت Youm در حد مطلوب، و برای نسبت Nattrass در حد قابل قبول تا نه چندان بد به دست آمد. دو نسبت از نظر بازگویی فروپاشی در مچ همخوانی ضعیف داشتند. نتیجه گیری اگرچه هیچ یک از دو روش مورد مطالعه، از نظر هدف مطالعه، دارای دقت در حد عالی نبودند ولی به نظر می رسد دقت روش Youm بالاتر باشد.

The purpose of this study was to assess the two popular methods of measurement of this angle with respect to inter- and intra-observer variability. Methods Good quality photographs were made of ten radiographs and two experienced orthopedists, in two separate occasions measured the length of third metacarpal bone, the longest length of capitates, and the distance between the base of third metacarpal bone and the articular surface of the radius. The remainder of the calculations was made by computer. Findings Intra-observer reliability of the Youm and Nattrass ratios were calculated as “high” and “fair”, respectively. With regard to inter-observer reliability, the accuracies of both indices were poor. Intra- and inter-observer agreement for detection of wrist collapse was “substantial” for Youm ratio and “fair to moderate” for Nattrass ratio. The ratios had poor agreement with each other, in regard to detection of collapse of the wrist joint. Conclusion Although neither of the ratios had perfect or high reliability with regard to the subject of the study, it seems that the accuracy of Youm index is higher than the other method.
