عنوان فارسی کارایی تکنیک های مبتنی بر رفتاردرمانی دیالکتیکی در کاهش مشکلات ناشی از درد در بیماران با سبک دلبستگی دل مشغول دارای درد مزمن عضلانی- اسکلتی: یک مطالعه تک موردی
چکیده فارسی مقاله سابقه و هدف: درد مزمن یک موقعیت تحلیل­ برنده و ناتوان کننده است. بنابراین، پژوهش حاضر با هدف بررسی نتایج اثربخشی تکنیک­ های رفتاردرمانی دیالکتیکی در کاهش فاجعه ­آمیزی، ترس از حرکت، ناتوانی ناشی از درد و شدت درد در بیماران با سبک دلبستگی دل­ مشغول مبتلا به درد مزمن عضلانی-اسکلتی انجام گرفت. مواد و روش­ ها: پژوهش حاضر به­ صورت تک­موردی بر پایه طرح خط پایه چندگانه انجام گرفت. آزمودنی­ های دو بیمار زن مبتلا به کمردرد مزمن بودند که به شیوه نمونه ­گیری هدفمند در مراجعه­ کنندگان به کلینیک درد تبریز انجام شد. ابزارهای غربال­گری، مورد مطالعه قرار گرفت. ابزارهای به­ کار رفته شامل پرسشنامه­ های فاجعه­ آمیزی (PCQ)، ترس از حرکت (TSK)، ناتوانی ناشی از درد (PDQ)، شدت درد (VAS) و مقیاس ­های روابط (RSQ) بود که در جلسات خط پایه، پنجم، هفتم، نهم، یازدهم، چهاردهم تکمیل شد. جهت تعیین تاثیر مداخله از روش ترسیم نمودار روند شدت علایم و میانگین کاهش خط پایه مربوط به پژوهش­ های تک­ موردی بهره گرفته شد. نتایج: یافته­ ها نشان داد که نمرات فاجعه­ آمیزی، ترس از حرکت، ناتوانی ناشی از درد و شدت درد هر دو بیمار از مرحله خط پایه به مرحله پیگیری کاهش یافت. و میانگین نمرات متغیرهای فوق به ­ترتیب (33/25 به 5/16)، (45/75 به 21)، (17/5 به 7) و (213/75 به 90) رسید. هم­ چنین، میانگین نمرات ایمنی دلبستگی در طول درمان از 1/25 به 2/3 افزایش یافت. نتیجه ­گیری: رفتار درمانی دیالکتیکی منجر به افزایش ایمنی دلبستگی و کاهش مشکلات مربوط به درد در بیماران با درد مزمن شد. لذا، در تحقیقات آتی این مطالعه در نمونه­ های با حجم بزرگ­تر توصیه می شود.

عنوان انگلیسی Efficacy of dialectical behavior therapy techniques in reducing problems of patients with preoccupied attachment style having chronic musculoskeletal pain: a single case study
چکیده انگلیسی مقاله Background: Pain is a degenerative and disabling condition which is accompanied by several psychological variables. Therefore, this study aimed to investigate the effectiveness of dialectical behavior therapy techniques in reducing the catastrophizing, fear of movement, pain-induced disability, and pain intensity in patients with preoccupied attachment style suffering musculoskeletal chronic pain. Materials and Methods: The Study was conducted as a single subject research based on multiple-baseline design. The participants were two women patients suffering chronic back pain that were selected through purposive method from among clients of Pain Clinic of Tabriz, based on pain specialist diagnosis, clinical interview and screening tools. Pain Catastrophizing Questionnaire, Tampa Scale for Kinesiophobia, Pain Disability Questionnaire, Visual Analogue Scale, and also relationships scales Questionnaire were filled by subjects in 5th, 7th, 9th, 11th, 14th baseline sessions and the follow up session. In order to study the treatment results drawing symptom severity diagram and mean base line reduction related to single subject studies. Results: Results indicate that average score of catastrophizing, fear of movement, pain disability, and pain intensity were decreased in both patients from baseline to follow up (33.25 to 16.5), (45.75 to 21), (17.5 to 7), and (213.75 to 90), respectively. In addition, during the treatment the average score of attachment security in both patients was increased from 1.25 to 2.3. Conclusion: Dialectical behavior therapy was effective in increasing secure attachment and decreasing pain-related problems in patients with musculoskeletal pain. Thence, it is recommended that the study be done with larger samples in future.
