عنوان فارسی مروری بر سیاست‏های تامین مالی در مسیر دستیابی به پوشش همگانی سلامت در ایران
چکیده فارسی مقاله مقدمه و اهداف: دسترسی بهنگام افراد به خدمات سلامت مورد نیاز بدون تحمیل هیچگونه بار مالی، جهت تحقق سلامت در جامعه بسیار ضروری است. از این رو، مطالعه حاضر با هدف مروری بر سیاست‏ها و برنامه‏های اتخاذ شده در حوزه تأمین مالی بخش سلامت انجام شد. روش کار: مطالعه کیفی حاضر ابتدا با رویکرد سیاست پژوهی به مرور قوانین و سیاست‏های بالادستی در حوزه تامین مالی سلامت در ایران پرداخت. سپس با برگزاری 7 جلسه گروه کاری متمرکز با مشارکت 28 نماینده از ذینفعان مختلف، سیاست‏ها و برنامه‌های این حوزه شناسایی شد. داده‏ها با استفاده از روش تحلیل چارچوب کدگذاری و طبقه‏بندی شد. نتایج: در پاسخگویی به بهبود وضعیت شاخص‏های حفاظت مالی، سیاست‏ها و برنامه‏های این حوزه در 10 مضمون در چهار طبقه جمع‏آوری منابع، مدیریت و انباشت منابع، تخصیص منابع و خرید راهبردی کالاها و خدمات شناسایی شد. از مهمترین سیاست‏ها و برنامه‌ها می‏توان به یکپارچه‏سازی صندوق‏های بیمه‏ای، افزایش منابع پایدار و تخصیص هدفمند یارانه‌ها، اولویت‏بندی خدمات سلامت، استقرار و بهبود نظام پرداخت مبتنی بر عملکرد با تأکید بر یکسان‏سازی جزءحرفه‏ای در بخش دولتی و خصوصی اشاره نمود. نتیجه‏گیری: در حوزه تأمین مالی نظام سلامت کشور، خلاء قانونی و سیاستی عمده‏ای وجود ندارد. آنچه به عنوان محدودیت عمده شناسایی شد، روش اجرا و تعهد به قوانین می‏باشد که چالش‏های اساسی در ارتباط با حفاظت مالی ایجاد کرده است و رفع آن مستلزم حمایت سیاسی کافی و ایجاد درک واحد بین ذینفعان در سطوح مختلف سیاستگذاری و اجرا‏ و انطباق طراحی برنامه‏های اجرایی با زیرساخت می‏باشد.

عنوان انگلیسی A Review of the Health Financing Policies Towards Universal Health Coverage in Iran
چکیده انگلیسی مقاله Background and Objectives: Timely access to required health services without any financial hardship is necessary to achieve public health. Therefore, the aim of present study was to review the policies and plans adopted in the health financing system. Methods: The qualitative study with a policy research approach reviewed upstream laws, policies, and plans in Iranian health financing. Then, by holding seven focused group discussions with 28 participants who represented various stakeholders, financing policies and programs were identified. Using framework analysis, the data were coded and categorized. Results: In response to health financial protection indexes improvement, policies and plans in this field were defined in 10 themes and 4 categories: resources collection, resources management and pooling, resources allocation, and strategic purchasing of health services. The most important policies and plans were integration of insurance funds, increasing sustainable financial resources and targeted allocation of subsidies, services priority setting, establishment and improvement of a performance-based payment system with emphasis on integration of the professional element in the public and private sectors. Conclusion: There is no gap for documented laws and policies in the Iranian health financing system. The main barriers are the method of implementation and adequate commitment to laws that have created major difficulties in relation to financial protection. Resolving these barrierrs requires sufficient political support and a common understanding between stakeholders at different levels of policymaking and implementation and designing programs based on the infrastructures.
