عنوان فارسی ارائه الگوی چالش های اداره امور بیمارستان های دولتی از دیدگاه ذینفعان: رویکرد ۱۸۰ درجه
چکیده فارسی مقاله سابقه و هدف: بررسی و شناسایی چالش‌‌های موجود در ساختار مدیریت بیمارستان‌‌ها که امور اجرایی را در تحقق اهداف با مشکل مواجه می‌‌نمایند، امری مهم و ضروری است. به‌طوری‌که در صورت عدم توجه کافی به پیامدهای ناشی از این چالش‌ها در مدیریت خدمات درمانی و بیمارستانی، ممکن است مشکلات جدی برای ذینفعان از جمله بیماران، کارکنان و مدیران ایجاد شود. از همین‌رو، در این مطالعه چالش‌های اداره امور بیمارستان‌های دولتی از دیدگاه ذینفعان شناسایی شده و راهکارهایی برای بهبود آن‌ها ارائه شده است.
مواد و روش‌ها: مطالعه حاضر از نوع آمیخته (کمی و کیفی) می‌باشد. جامعه مورد مطالعه دو گروه از افراد (کلیه ارائه‌دهندگان خدمات سلامت و مدیران/خبرگان بیمارستان‌ها) شامل 14 نفر خبره در بیمارستان دولتی بودند. در بخش کیفی، با استفاده از رویکرد مصاحبه نیمه‌ساختاریافته، چالش‌های بیمارستان‌های دولتی مشخص شده و در بخش کمی با تهیه پرسشنامه بر اساس یافته‌های بخش کیفی و نظرات 363 نفر از کارشناسان، اقدام به آزمون عوامل با تحلیل عاملی اکتشافی و با استفاده از نرم‌افزارهای SPSS و AMOS نسخه 24 شده است.
یافته‌ها: چالش‌های اداره امور بیمارستان‌‌های دولتی از نگاه ذینفعان در چهار دسته چالش‌های مالی، مدیریت فرآیندی و تصمیم‌گیری، مدیریت منابع انسانی و زیرساختی طبقه‌بندی شده است. نتایج تحلیل داده‌ها نشان می‌دهد چالش مدیریت فرآیندی و تصمیم‌گیری با ضریب استاندارد 0/89 و معنی‌داری 7/881، چالش‌های مالی با ضریب استاندارد 0/79 و معنی‌داری 6/994، چالش مدیریت منابع انسانی با ضریب 0/75 و معنی‌داری 6/641 و چالش زیرساختی با ضریب 0/43 و معنی‌داری 3/807 محاسبه شده‌اند.
استنتاج: نتایج مطالعه نشان می‌دهد چالش‌های مدیریت فرآیندی و تصمیم‌گیری در بیمارستان‌های دولتی به ‌عنوان یک عامل مهم در تحقق اهداف عملیاتی بیمارستان، نمود داشته و سایر عوامل مؤثر در خدمات‌رسانی بیمارستان‌های دولتی را تحت تأثیر قرار می‌دهد. هم‌چنین چالش‌های مالی، مدیریت منابع انسانی و زیرساختی باعث کاهش ظرفیت پاسخ‌گویی بیمارستان‌ها از نظر کمی و کیفی به نیاز بیماران و اهداف سازمانی می‌شوند.

عنوان انگلیسی Modelling the Challenges of Public Hospital Management: A 180-Degree Stakeholder Perspective
چکیده انگلیسی مقاله Background and purpose: This study aims to examine the challenges within the management structures of public hospitals, particularly how these challenges impede the achievement of executive goals. If the consequences of these challenges are not adequately addressed, they may result in significant issues in the management of medical and hospital services. These issues could have a detrimental impact on various stakeholders, including patients, hospital staff, and administrators. Therefore, this research identifies the key administrative challenges faced by public hospitals from the perspective of multiple stakeholders and proposes potential solutions to improve hospital management and service delivery.
Materials and methods: This mixed-method study (quantitative and qualitative) involved two primary groups: healthcare providers and hospital managers/senior professionals, including 14 senior staff from a public hospital. In the qualitative phase, semi-structured interviews were conducted to identify the challenges in public hospitals. In the quantitative phase, a questionnaire was developed based on the qualitative findings and the opinions of 363 experts. Exploratory factor analysis was performed using SPSS and AMOS software (version 24).
Results: According to stakeholders, the challenges in managing public hospitals were identified across four key categories: financial, procedural and decision-making, human resources, and infrastructure. The results of the data analysis revealed that procedural and decision-making challenges had a coefficient of 0.89 with a significance level of 7.881; financial challenges had a coefficient of 0.79 with a significance level of 6.994; human resource challenges had a coefficient of 0.75 with a significance level of 6.641; and infrastructure challenges had a coefficient of 0.43 with a significance level of 3.807.
Conclusion: The study results indicate that procedural and decision-making challenges in public hospitals are key factors significantly affecting the achievement of hospital operational goals and have a broad influence on other critical aspects of public hospital services, including patient satisfaction and service efficiency. Additionally, financial constraints, human resource limitations, and infrastructure challenges diminish hospitals' capacity to effectively meet patients' needs and organizational goals in both quantitative and qualitative terms, posing serious barriers to healthcare quality and accessibility.
