Frontal T-P Angle: A Novel ECG Parameter for Predicting SYNTAX-1 and SYNTAX-2 Scores in Acute Non–ST-Segment Elevation Myocardial Infarction

Background and objective: Sparse number of studies are present on P-wave and T-wave axes, most of which are on mortality. Frontal T-P angle(fT-Pa) is a novel ECG-derived parameter of ventricular repolarization. Our objective was to evaluate the association between this novel parameter and the Syntax-1 and -2 scores (SS-1 and SS-2) in patients with acute non-ST-segment elevation myocardial infarction (ANSTEMI). Methods: The study was conducted retrospectively on a total of 158 ANSTEMI patients undergoing coronary angiography. Study population was subdivided on the basis of 22
22 and <26.2SS-2>26.2. fT-Pa was calculated by subtracting P-wave axis from T-wave axis, which were obtained from the built-in software system of ECG device, and was compared between the groups. Results: fT-Pa was significantly correlated to age (r=0.242, p=0.003), SS-2 (r=0.229, p=0.005) and T-wave axis (r=-0.626, p<0.001). Age and fT-Pa was significantly correlated to and associated with SS-2 [Beta: 0.679, OR: 0.700 (0.584-0.816), p<0.001 for age] [Beta: 0.147, OR: 0.048(0.012-0.085), p=0.010 for SS-2]. ROC-curve analysis gave a cut-off value of 36.5 with 64% and 68% specificity (AUC: 0.674, p<0.001) for fT-Pa to predict SS-2 greater than 26.2, and another cut-off value of 39.5 with 70% sensitivity and 62% specificity (AUC:0.692, p=0.010) to predict SS-1 greater than 22. Conclusion: fT-Pa correlates better to and is associated better with SS-2 as compared with SS-1. fT-Pa could be a simple, reproducible and useful parameter in the prediction of the complexity of CAD even in patient populations having normal TWA and PWA ranges.
