P-5: Serum Laminin Concentrations As Potential Tumor Markers for Upper Gastrointestinal Cancers

Objective: Nowadays early detection of patients with upper gastrointestinal (UGI) cancer is a great problem so most of these patients are diagnosed in advanced metastatic stages. It was shown that the serum laminin (LN) levels increases in various malignant tumors. Materials and Methods: We aimed to determine whether the serum levels of LN can discriminate esophageal squamous cell carcinoma (ESCC), cardiac gastric cancer (CGC) and non cardiac gastric cancer(NCGC) patients from patients without UGI cancer (as control) and to find the optimal cut-off values for discrimination of patients and controls. Concentrations of LN in serum were measured by enzyme-linked immunosorbent assay in 20 CGC, 23 NCGC and 20 ESCC incident cases and 25 controls. Results: Serum levels of LN between the controls (58.3±14.9ng/ml), CGC (86.3±37.7ng/ml), NCGC (68.1±14.7ng/ml) and ESCC patients (69.4±11.9ng/ml) were statistically differed (p< 0.05). Cut-off values of 62.0, 54.9 and 56.7ng/ml LN for discrimination of the CGC, NCGC and ESCC patients from control, showed acceptable AUC (CGC: 0.826, NCGC: 0.671, ESCC: 0.706, p< 0.05), accuracy (CGC: 73.7%, NCGC: 62.5%, ESCC: 64.4%), sensitivity (CGC: 90.0%, NCGC: 82.6%, ESCC: 85.0%), specificity (CGC: 60.0%, NCGC: 44.0%, ESCC: 48.0%), positive (CGC: 64.2%, NCGC: 57.5%, ESCC: 56.6%) and negative predictive value (CGC: 88.2%, NCGC: 73.3%, ESCC: 80.0%), respectively. Conclusion: Our study suggests that LN may be used to identify potentially high-risk groups of upper gastrointestinal cancers for further diagnostic workups, particularly in high incidence areas and maybe measurement of this glycoprotein can be a useful tool for early detection of these diseases.
