I-21: Predictive Pathology: Facilitating the Way to Personalized Medicine

Objective: This is an attempt to highlight pathologists relatively new but highly demanding role to analyze bio-specimens particularly from cancer patients to predict what therapy will most likely suit to a particular patient. Materials and Methods: This study includes predictive markers for breast cancer, gastric cancer, colonic cancer, lymphomas & leukemias. The predictive markers analysed were Estrogen & Progesterone receptor (ER&PR) by immunohistochemistry (IHC) in breast carcinoma. Her-2/neu oncogene analyses by IHC & FISH in breast and gastric carcinoma. DNA ploidy and S-phase fraction in breast carcinoma. C-kit by IHC in gastro-intestinal stromal tumours (GIST), BCl2, Bcl6 & P53 in diffuse large B-cell lymphoma (DLBCL) and Zap-70 in chronic lymphocytic leukemia (CLL). Results: Approximately 60% of breast carcinoma in Pakistani patients shows ER positivity, 40% show PR positivity, these patients get anti-estrogen therapy, 20% show Her-2/neu gene amplification in breast carcinoma and 12% gastric carcinomas show Her-2/neu gene amplification. These patients receive herceptin. DNA ploidy analyses was not of much value in breast cancer, however S-Phase fraction analyses in breast cancer was found to be of great value All GIST express C-kit and receive Gleevec. In DLBCL P53 mutation was shown by us to be significantly correlated with decreased disease-free and overall survival. In CLL strong Zap-7- expression was associated with poor survival. Conclusion: Predictive markers for individualized care in particular for cancer management are mandatory for some cancers like ER, PR & Her-2/neu in breast cancer while in others like Lymphoma they are highly desirable. In our study various differences in terms of frequency of various markers was noted compared to data from western world.
