عنوان فارسی ضرورت بازنگری لاگ بوک در بخش های بالینی دانشگاه علوم پزشکی بیرجند
چکیده فارسی مقاله آموزش بالینی را می توان به صورت فعالیتی تسهیل کننده برای یادگیری در محیط بالینی دانست که در طی آن دانشجویان با حضور بر بالین بیمار تجربیاتی کسب می کنند و در تعامل با استاد و محیط مفاهیم آموخته شده را در عمل به کار می گیرند. در آموزش بالینی فرصتی برای دانشجو فراهم می آید تا دانش نظری خود را به مهارت های ذهنی و روانی حرکتی متنوعی که برای مراقبت از بیمار ضروری است تبدیل کند. ارزشیابی فرآیند یاددهی و یادگیری از فعالیت های مورد توجه در موسسات آموزشی است زیرا تربیت نیروی انسانی متبحر، کارآزموده و دارای صلاحیت به میزان زیادی وابسته به این فرآیند است. بکارگیری تکنیک های نوین آموزشی، افزایش بهره وری مراکز آموزشی و در نهایت ارتقاء کارآیی، هدفی است که در تمامی مراکز آموزشی دنیا دنبال می شود اما در مورد رشته های علوم پزشکی که بطور مستقیم با حفظ و ارتقاء سلامت مردم و جامعه در ارتباط است از توجه بیشتری برخوردار است و سعی بر آن است تا با استفاده از روش های جدید بتوان میزان اثربخشی آموزش پزشکی را بهتر نمود. یکی از شیوه های ارزشیابی که دارای ساختار بوده و بر تجربیات یادگیری در طول دوره آموزش متمرکز می باشد لاگ بوک یا آموزش نامه است. آموزش نامه وسیله ساده ای برای فراگیران، چارچوبی جهت سازماندهی و ثبت فعالیت های آموزشی مختلف آنان می باشد. یکی از خصوصیات منحصر به فرد آموزش نامه، موظف کردن دانشجویان به انجام تعداد مشخصی مهارت است. این مشخصه موجب می شود دانشجویان وظایف خود را به صورت برنامه ریزی شده و هدفمند انجام دهند و با تکرار و تمرین از بروز اشتباهات جلوگیری کنند. در حال حاضر بسیاری از جنبه های مرتبط با آموزش بالینی نسبت به گذشته تفاوت نموده است. توسعه ی فناوری، تغییرات الگوی زندگی فردی یا همان سبک زندگی و مواجهه بیشتر با عوامل خطر ساز محیطی( مانند توسعه صنایع شیمیایی، مواد شیمیایی طبیعی یا مصنوعی، عوامل بیولوژیک شامل ویروسها و سایر میکروارگانیسمها، عوامل رفتاری شامل تنش های روانی و اعتیاد دارویی) باعث تغییر در الگوی بروز بیماری ها به طور اعم و به ویژه در زمینه بیماریهای واگیر و غیرواگیر شده است. بنابراین متناسب با این تغییرات انتظار می رود مهارت ها و توانمندی های فارغ التحصیلان و دانش آموختگان نیز ارتقاء یابد. از آنجایی که شناخت و تحلیل اثربخشی نیاز‏های آموزشی، پیش نیاز یک سیستم آموزشی موفق است، تجدیدنظر و بازنگری مستمر برنامه­های آموزشی مبتنی بر امکانات، شرایط و نیازهای جدید امری اجتناب­ناپذیر می باشد . در همین راستا، نتایج مطالعه ای که بر روی 233 نفر از دانشجویان پزشکی مقطع بالینی( کارآموز و کارورز) دانشگاه علوم پزشکی بیرجند در سال تحصیلی 96-1395 انجام شد نشان داد که 5/24 درصد دانشجویان انجام و تکمیل فعالیت های لاگ بوک را در یادگیری مهارتهای بخش بالینی در حد زیاد و خیلی زیاد مفید دانستند و 8/70 درصد، ضرورت بازنگری لاگ بوک های موجود در بخش های بالینی را بیان کردند. بنا بر نتایج مطالعه ای که در بالا بدان اشاره شد ضرورت بازنگری لاگ بوک های مورد استفاده در بخش های بالینی دانشگاه علوم پزشکی بیرجند براساس آخرین نکات بالینی، بیماریها شامل بیماریهای بازپدید و نوپدید و روشهای تشخیصی درمانی متناسب با تغییرات در گایدلاین های تشخیصی- درمانی با استفاده از جدیدترین منابع احساس می شود. همچنین با توجه به استفاده فراگیر از لاگ بوک های الکترونیک در سالیان اخیر در دانشگاههای علوم پزشکی سراسر کشور که در راستای تحول و نوآوری در آموزش علوم پزشکی می باشد، پیشنهاد می شود لاگ بوک ها در این دانشگاه نیز به صورت الکترونیک ارائه و ارزشیابی شوند. لاگ بوک الکترونیک با ثبت مداوم فعالیت ها و تکنیک های بالینی بر اساس اهداف آموزشی، علاوه بر تثبیت یادگیری موارد عدم دستیابی به اهداف بالینی را مشخص می کند و به این طریق نظارت و ارزشیابی دانشجو به طور منظم صورت می گیرد .

چکیده انگلیسی مقاله Clinical education can be a facilitator activity for learning in the clinical setting which during students gain experiences with the patient's bedside and interact with the teacher and environment they taught concepts apply in practice. Clinical education provides an opportunity for student until transform its theoretical knowledge to diverse intellectual and psychomotor skills that is essential to patient care. The evaluation process of teaching and learning is one of the noticeable activities in educational institutions because the training of skilled manpower trained and competent to a large extent is dependent on this process. The use of modern educational techniques, increase productivity and ultimately enhance the effectiveness of training centers is the goal that all training centers around the world looking for but, in the field of medical sciences that are directly associated with maintaining and improving the health of people and society is more attention and tried to use new methods to improve the effectiveness of medical education. One of the methods of evaluation that has been structured and focused on learning experiences during training is logbook. Logbook is a simple means for learners, a framework for organizing various educational activities and records them. One of the unique characteristics of a logbook, students is required to doing a certain number of skills. This characteristic cause to students does their tasks as planned and purposeful and with repetition and practice to prevent errors. At present, a lot of aspects related to clinical education are different than in the past. The development of technology, changes in the pattern of an individual's life or lifestyle and higher exposure to environmental risk factors (Such as the development of the chemical industry, natural or synthetic chemicals, biological agents including viruses and other microorganisms, behavioral factors including psychological stress and drug addiction) cause to change in the pattern of diseases in general and particularly in the field of communicable and non-communicable diseases. Therefore, according to these changes are expected skills and abilities of graduates will also be promoted. Since, identification and analysis of the effectiveness of training needs is a prerequisite for a successful education system, revision and continuous review of training programs based on the possibilities, conditions and new needs it is inevitable. In this regard, the results of a study on 233 medical students in clinical stage (Astazhry or externs and interns) Birjand University of Medical Sciences in the academic year 2016-2017 showed that 24.5% of students knew high or very high helpful completion of the log books activities in learning of clinical wards skills and 70.8% expressed the necessity of reviewing available log books in clinical wards. According to the results of the above-mentioned study is felt necessity of reviewing log books used in clinical wards of Birjand University of Medical Sciences according to the latest clinical issues, diseases including emerging and reemerging diseases and diagnostic -therapeutic methods in accordance with changes in diagnostic - therapeutic guidelines using the newest resources. Futhermore, due to the pervasive use of electronic log books in recent years in medical universities across the country that are in line with developments and innovations in medical education, it is recommended that the log books at this university also, present and evaluate electronic. Electronic log books with ongoing record of activities and clinical techniques based on educational goals, in addition to learning consolidation determine the lack of measures to achieve clinical objectives and in this way take place regular monitoring and evaluation of student.
