عنوان فارسی ارزیابی مراقبت‌های پرستاری مرتبط با ایمنی بیمار با استانداردها در بخش مراقبت‌های ویژه(ICU)
چکیده فارسی مقاله چکیده زمینه و هدف: ارتقاء کیفیت خدمات درمانی از دغدغه‌های اصلی سازمان‌های ارایه دهنده خدمات سلامت است. امنیت بیمار به معنای فقدان هرگونه آسیب ناشی از ارایه خدمات بهداشتی و درمانی است. در بخش مراقبت‌های ویژه به دلایلی چون پیچیدگی شرایط بیمار و فرایند درمان، امکان بروز خطا و رویدادهای ناخواسته به حداکثر می‌رسد و نیازمند توجه جدی است. هدف از این مطالعه تعیین میزان مطابقت مراقبت‌های پرستاری مرتبط با ایمنی بیمار با استانداردها  در بخش‌های مراقبت ویژه بود.   روش بررسی: در این مطالعه توصیفی، 1998 مراقبت پرستاری  مرتبط با ایمنی بیمار طی عملکرد شیفت صبح 54 پرستار در سه   بخش مراقبت ویژه بیمارستان های منتخب دانشگاه علوم پزشکی یاسوج در زمستان سال   1394  با استفاده از چک لیست پژوهشگر ساخته که شامل 37 عبارت  و 6 حیطه بود، ارزیابی شد. این حیطه‌ها شامل پذیرش  و ارزیابی اولیه بیمار، انجام پروسیجر صحیح در محل صحیح از بدن بیمار، اجتناب از اتصالات نادرست لوله‌ها و سوند بیمار، پیشگیری از سقوط بیمار، پیشگیری از زخم بستر و پیشگیری از عفونت‌های  بیمارستانی بود. چک لیست پس از تعیین روایی صوری و محتوا(96 درصد=CVI) و پایایی به روش محاسبه ضریب توافق بین مشاهده گران(99 درصد= ICC)، با مشاهده عملکرد توسط پژوهشگر در شیفت‌های صبح، برای 54 پرستار تکمیل و داده‌ها با استفاده از آمار توصیفی بررسی شد.   یافته‌ها: در مشاهده عملکرد 54 پرستار بخش‌‌های مراقبت ویژه ، متوسط میزان مطابقت مراقبت‌های پرستاری مرتبط با ایمنی بیمار در مقایسه با چک لیست مورد نظر از 100 امتیاز، نامطلوب(75/46)ارزیابی شد. بیشترین میزان مطابقت مربوط به حیطه پیشگیری از سقوط بیمار نسبتاً مطلوب(25/50) و کمترین میزان مربوط به حیطه پیشگیری از زخم بستر نامطلوب(87/1) ارزیابی گردید.     نتیجه‌گیری: میزان مطابقت مراقبت‌های پرستاری مرتبط با ایمنی  بیمار در بخش مراقبت‌های ویژه، با استانداردها فاصله دارد. مدیران حوزه درمان در راستای ارتقای کیفیت خدمات درمانی ارایه شده به بیمار به وسیله پرستار در رابطه با شاخص های ایمنی بیمار در بخش مراقبت ویژه و به دنبال آن افزایش ایمنی بیماران جهت جلوگیری از آسیب رسیدن به آنها در نتیجه ارایه این مراقبت‌ها، باید  بر حسابرسی و نظارت منظم در مراقبت‌های پرستاری و برنامه‌های آموزش مداوم، تأکید بیشتری نمایند.   ارزیابی، ایمنی بیمار، بخش مراقبت‌های ویژه، مراقبت پرستاری،

عنوان انگلیسی Evaluation of Patient-Related Nursing Care with Standards in Intensive Care Unit (ICU)
چکیده انگلیسی مقاله Abstract   Background & aim: Improving the quality of health care services is one of the main concerns of organizations providing health services. Patient safety means is the absence of any damage caused by the provision of health services. In the intensive care unit, due to complications of the patient's condition and the treatment process, the chance of occurrence of unexpected errors and events is maximized and serious attention is required . The purpose of this study was to determine the level of compliance of patient-related nursing care with standards in intensive care units.   Methods: In this descriptive study, 1998 nursing care associated with patient safety were assessed during the morning shift function of 54 nurses in three Intensive care unit of selected hospitals of Yasuj University of Medical Sciences using a researcher-made checklist containing 37 terms and 6 domains in winter 2015. These areas include the initial acceptance and evaluation of the patient, the correct procedure in the correct location of the patient's body, avoiding inappropriate connections between the tubes and the patient's catheter, preventing the patient falling down, preventing ulcer and nosocomial infections. The checklist after determining the face validity and content (96% = CVI) and reliability by means of calculating the agreement coefficient between viewers (99% = ICC), via observing the performance by the researcher in morning shifts was completed for 54 nurses. Collected data were analyzed using Descriptive statistics.   Results: In view of the performance of 54 nurses in intensive care units, the average level of compliance of patient-related nursing care in comparison with the checklist was less than 100 points which was unfaroble(46.75). The highest correlation with the patient's avoidance domain was relatively favorable (50.25) and the lowest level was related to the area of ​​undesirable subsoil prevention (1.87).   Conclusion: The compliance rate of patient-related nursing care in ICU is far from standards. The managers of the area of ​​treatment in order to improve the quality of nursing care provided by the nurse in relation to patient safety indicators in the intensive care unit and, consequently, to increase the safety of patients in order to prevent their injury as a result of the provision of these care, and regular monitoring of nursing care and continuing education programs will be more emphasis.
