عنوان فارسی تاثیر استقرار طرح تحول نظام سلامت بر پرداخت رسمی بیماران بخش قلب: مطالعه موردی
چکیده فارسی مقاله مقدمه: سلامت از جمله حقوق طبیعی و نیازهای اساسی انسان ها بوده و به عنوان یکی از پیش شرط های اصلی نظام های رفاه اجتماعی شناخته شده است. با این وجود نبود محافظت مالی در سلامت به عنوان یکی از بیماری های نظام های سلامت شناخته شده است. پس از روی کار آمدن دولت یازدهم، وزارت بهداشت، درمان و آموزش پزشکی طرح تحول سلامت را به دنبال محافظت مالی از مردم تدوین کرد. روش ها: پژوهش حاضر، مطالعه ای مقایسه ای از نوع مقطعی می باشد. جامعه آماری پژوهش صورتحساب کلیه بیماران بخش قلب مرکز در بازه زمانی مشخص شده بودند. با استفاده از جدول مورگان و روش نمونه گیری تصادفی طبقه ای به ترتیب تعداد 120 و 154 نمونه از پرونده های بخش قلب در سال های 92 و 93 انتخاب شد. داده های مورد نیاز از صورتحساب مالی بیماران استخراج و با استفاده از روش آماری t-test و من ویتنی تحلیل شد. برای تحلیل داده ها از نرم افزار SPSS v.22 استفاده گردید. یافته ها: با افزایش زیاد تعرفه های خدمات درمانی طی اجرای طرح، میزان پرداخت رسمی بیماران کاهش چشمگیری نداشته است. همچنین، سازوکارهای بیمه ای مانع از رسیدن طرح تحول به هدف کاهش میزان پرداخت مستقیم بیمار شده است. نتیجه گیری: نتایج مطالعه بیانگر تأثیر عوامل مختلفی همچون سازوکارهای بیمه ای، تعرفه خدمات درمانی، نوع بیمه فرد، سرپایی یا بستری بودن می باشد؛ بنابراین، شایسته است دولت نتایج حاصل از مطالعه را در برنامه ریزی به منظور افزایش پوشش و محافظت مالی مردم به کار گیرد.

عنوان انگلیسی Impact of Health System Reform on Patients’ Formal Payment in Heart department: Case Study
چکیده انگلیسی مقاله Introduction: Health is one of the natural rights and basic human needs and is recognized as one of the prerequisites for social welfare systems. However, Lack of financial health protection is recognized as one of the diseases in health systems. When the eleventh government get start, the Ministry of Health developed the health sector reform to seek financial protection from the people. The purpose of this study was to investigate the effect of Health System Reform on Patients' Formal Payment in Heart Sector of Qazvin's velayat Hospital. Methods: This was a comparative study of discontinuous time series. The bills of all patients referring to the heart department during the specified period formed the statistical population of the study. By using Morgan table and stratified random sampling, 120 and 154 samples from cardiac files were selected in the years 2013 and 2014 respectively. Data were extracted from patient's financial bills.  Finally, data was analyzed by t-test and SPSS software. Results: With the increase in health care tariffs during the plan implementation, the official payment of patients has not declined significantly, and new insurance mechanisms have prevented the development plan from reaching the goal of reducing the patient's direct payment to six percent of the bill. Conclusion: Results of the study indicate the impact of various factors such as insurance mechanisms, health care tariffs and kind of patient insurance. Therefore, it is worth the government to use results of the study in planning to increase coverage and financial protection of the people.
