عنوان انگلیسی Maternal and Fetal Doppler Blood Flow Velocimetry Changes in the Management of Asymptomatic Preterm Labor With Vaginal Progesterone Tablet
چکیده انگلیسی مقاله In order to assess the Maternal and Fetal Doppler blood flow velocimetry changes in the management of asymptomatic preterm labor with vaginal progesterone tablet, it is found that the use of vaginal tablet of progesterone affects uterine, umbilical and fetal middle cerebral arteries in the second and third trimesters of pregnancy who were compromised preterm labor. In this prospective both before and after clinical trial study wasconducted in the outpatient clinic and emergency unit in Moheb Yas hospital along years 2015-2016. For all subjects, 200 mg vaginal progesterone tablet (one tablet before bedtime) were administered. Additionally, transvaginal sonography was performed to measure uterus, umbilical and middle cerebral arteries Doppler before the treatment and 24 hours after the treatment respectively with progesterone. The correlation between Doppler parameters and gestational age assessed using univariate analysis. The mean gestational age of the study population was 28.8 ±2.9 weeks, ranging from 23 to 32 weeks gestation. There was a strong correlation between the Middle Cerebral Artery Pulsatility Index (MCA-PI) and gestational week before progesterone administration in the studied age range (16–32 weeks, R = 0.668, P = 0.005), and so there was a strong correlation between right Uterine Artery Resistance indice Index (UtA-RI) and gestational week before progesterone administration (R = - 0.516, P = 0.034). There was no significant change in the fetal middle cerebral arterial-peak systolic velocity (MCA-PSV), middle cerebral artery resistance index ( MCA-RI), MCA-PI, Umbilical arterial Systolic-Diastolic (UA S/D), Right UtA-RI and Left UtA-RI before the beginning of progesterone treatment to after. Our study demonstrates that the use of 200 mg vaginal tablet of progesterone for treatment of preterm labor led to a reduction in the middle cerebral artery and the uterin artery PI and RI and an increase in umbilical artery S/D respectively. Vaginal progesterone can improve fetoplacental perfusion in pregnancies complicated by preterm labor. The safety, and possible additional benefits of this intervention needs to be investigated in further studies on the impact of progesterone’s vasodilatory effect within the maternal–fetal circulation.
