عنوان فارسی طراحی و ارزشیابی "فرم ثبت آموزش ‌بیمار" برای ثبت ساختارمند فرآیند آموزش ‌بیمار
چکیده فارسی مقاله مقدمه: ثبت آموزش ‌بیمار، ابزاری مهم جهت تعیین صحت مداخلات آموزشی، حمایت و حفظ حقوق قانونی گیرندگان و ارائه‌دهندگان خدمات است. هدف این پژوهش طراحی و ارزشیابی "فرم ثبت آموزش ‌بیمار" برای ثبت ساختارمند فرآیند آموزش ‌بیمار بود. مواد و روش‌ها: مطالعه‌ی روش‌شناسی حاضر ‌در مرکز آموزشی درمانی الزهرا (س) اصفهان طی سال‌های 1390 تا 1392 انجام شده است. مشارکت‌کنندگان شامل 256 کارشناس و متخصص بالینی بودند که از طریق نمونه‌گیری هدفمند در سه گروه مرکزی، راهبردی و فرآیندی/بازخوردی همکاری نمودند. دو مرحله طراحی و ارزشیابی فرم ثبت آموزش بیمار با 24 دوره نظرخواهی اولیه قبل از اجرا و جمع-آوری 49 یادداشت‌ در عرصه، 11مصاحبه و 52 گزارش و 6 دوره نظرخواهی مجدد طی 9 ماه اجرای آزمایشی فرم اولیه انجام شد. ارزشیابی با تحلیل محتوای کیفی قراردادی بازدارنده‌ها و مزایای ثبت و اجرای آموزش بیمار تا اشباع داده‌ها و تصویب فرم بازنگری‌شده ادامه یافت. یافته‌ها: فرم اولیه‌ی ثبت آموزش بیمار به صورت ساختارمند با یک سربرگ مشخصات بیمار به همراه 4 جدول ثبت آموزش‌‌های بدو ‌ورود تا موقع ترخیص در 9 ستون با زیرنویس‌ ثبت اختصارات کامل فرآیند آموزش بیمار طراحی شد. فرم بازنگری‌شده پس از اجرای آزمایشی شامل اختصارات کم‌تر به همراه چهارجدول ارزیابی اولیه، آموزش موقع پذیرش و آموزش‌های‌ حین‌بستری و مراقبت در منزل توسط دو گروه کارشناسان بالینی و پزشکان بود. طراحی و توصیه‌های دارو، به کارت ترخیص منتقل شد. بازدارنده‌های ثبت و آموزش بیمار شامل پیچیدگی و عدم آشنایی کارشناسان با فرم اولیه و همچنین کمبود وقت، کمبود نیرو، کمبود مهارت و محتوای کمک ‌آموزشی و مزایای مربوطه برای سازمان، بیماران و کارشناسان بالینی در دو طبقه با 6 کد اصلی و 43 کد فرعی تبیین گردید. نتیجه‌گیری: طراحی "فرم‌ ثبت آموزش بیمار"، منجر به تسهیل ثبت و اجرای کامل فرآیند آموزش بیمار شد. فرم طراحی شده می تواند برای ثبت آموزش بیمار در دیگر مراکز درمانی مفید واقع شود.

چکیده انگلیسی مقاله Introduction: Documenting patient education (PE) in patient's records is an important tool for validating educational interventions to support the legal rights of both patient and clinical experts. The aim of this study was development and evaluation of a structured "Patient Education Record" for complete documentation of PE process. Materials & Methods: This methodological research, carried out in Alzahra University Hospital in Isfahan (2012 to 2014). Through organizing three groups of central, steering and feedback groups and purposeful sampling, 256 professionals and experts participated in this study. Two stages of development and evaluation of the form was conducted with gathering 24 series of primary participants' feedbacks and viewpoints consisting 49 field notes, 11 interviews and 52 reports during 9 months pilot period accompanied with six series of secondary feedbacks. Qualitative content analysis was done for evaluation of advantages and inhibitors of the form until achieving data saturation and approval revised form. Findings: Primary PE record, developed in a structured format with a header of patient's profile, followed by four tables of educational services from admission to discharge time with 9 columns of subtitles for filling in full PE process through footnote abbreviations. Revised form developed during pilot period, consists fewer abbreviations with four tables of initial assessment as well as teaching records from admission to home care by both clinical professionals and physicians. Drugs considerations and recommendations transferred to discharge card. Education inhibitors consist of the complexity and disorientation of clinical professionals with primary form as well as lack of time, manpower, skill and educational media. Advantages of PE record for the organization, patients, and clinical experts were explained in two categories, six axial codes and 43 secondary codes. Conclusion: "Patient Education Record", facilitated complete documentation and implementation of PE process. The developed form would be useful for recording patients' education in other hospitals
