عنوان فارسی فراوانی خطا‌های گزارش شده در بیمارستان‌های وابسته به دانشگاه علوم پزشکی کرمان طی سال‌های ۹۷-۱۳۹۳: یک گزارش کوتاه
چکیده فارسی مقاله زمینه و هدف: خطاهای درمانی تهدید مهم ایمنی بیمار در نظر گرفته می­شوند. راهبرد مؤثر برای کنترل خطاهای پزشکی، گزارش­دهی به موقع و تحلیل آن­ها می­باشد. پژوهش حاضر با هدف تعیین نوع و فراوانی خطاها در مقطع زمانی 5 ساله در بیمارستان­های آموزشی کرمان انجام گردید. مواد و روش‌ها: این پژوهش از نوع مقطعی و جامعه بیمارستان‌های آموزشی شهر کرمان بود. حجم نمونه تمام خطاهای گزارش­شده به­صورت سرشماری بودند، روش­های آماری توصیفی به کار برده شد. یافته­ها: بیش‌ترین آمار گزارش­دهی خطاها در سال 1397 بود. واحدهای پاراکلینیک و پشتیبانی، نوبت­کاری شیفت صبح و پرستاران بیش‌ترین میزان گزارش خطا را داشتند. موارد منجر به آسیب جدی، 6/5 درصد بوده است. نتیجه­گیری: در طول سال‌های مطالعه در مراکز مذکور 2326 خطا استخراج گردید.گزارش خطاها در بیمارستان­های مورد مطالعه وضعیت مطلوبی ندارد. لزوم ارتقاء فرهنگ ایمنی در بیمارستان­ها اقدامی اساسی جهت مدیریت خطاست. واژه­های کلیدی: خطا، ایمنی بیمار، خطای درمانی، بیمارستان، کرمان

عنوان انگلیسی Frequency of Errors Reported in Hospitals Affiliated to Kerman University of Medical Sciences during 2014-2018: A Short Report
چکیده انگلیسی مقاله Background and Objectives: Therapeutic errors are considered an important threat to the patient safety. An effective strategy for controlling medical errors is timely reporting and analysis. The aim of this study was to determine the type and frequency of errors over a period of 5 years in Kerman teaching hospitals. Materials and Methods: This is a cross-sectional research with the population of all educational hospitals of Kerman Medical Sciences University. Sample size was all reported errors reviewed by census. Descriptive statistics were used. Results: The highest number of error reporting statistics was in 2018. Paraclinical and morning shift support units and nurses had the highest error reporting rate. Cases leading to serious injury were 5.6%. Conclusion: During the study years, 2326 errors were extracted in the mentioned centers. Reporting errors in the studied hospitals is not favorable. The need to promote a safety culture in hospitals is a fundamental step in error management.
