Effect of Mitral Valve Prolapse on Heart Rate Variability and Autonomic Nervous System in Children

Background: It has been shown that patients with mitral valve prolapse (MVP) may reveal symptoms of autonomic dysfunction and also heart rate variability (HRV) is often used as a noninvasive marker of autonomic activity. The purpose of this study is to explore HRV and autonomic nervous system in children with MVP. Methods: The population of this cross-sectional study consisted of 60 healthy children in the age range of 5–15 years as well as 60 patients with MVP. To measure the length and dispersion of the P-wave and QT interval, surface 12-lead electrocardiograms (ECGs) was used. For the analysis of heart rate variability, both time-domain and frequency-domain analyses of Holter monitoring ECGs were performed. Electrocardiogram and echocardiography were carried out by two cardiologists. Results: The mean age of patients was 13.12 ± 1.50 years in the intervention group and 13.20 ± 1.81 years in the control group (p>0.05). The two groups were not significantly different in terms of age, gender, height, weight, body mass index (BMI), systolic and diastolic blood pressures, and heart rate. The sample consisted of 25 subjects with mild MR, 20 with moderate MR and 15 with severe MR. The maximum duration and P-wave dispersion of the P-wave in patients with MVP were significantly different from the healthy children (p=0.001, respectively). The longest and shortest QT dispersion and QTc were significantly different between the two groups (p=0.004, p=0.043, p=0.001, p=0.001, respectively). Conclusion: The results of this study indicated inhomogeneous depolarization in atrial tissues of the children with MVP. Elevated QT dispersion and QTC exhibit symptoms of ventricular arrhythmias in children with MVP. Further, heart rate variability and autonomic nervous system were conflicts in the MVP group.
