عنوان فارسی راهکارهای اصلاحی ارتقاء عملکرد نظارت و بهبود زیر ساختهای اجرای دستورالعمل طرح جدید کارانه۱۳۹۴: یک مطالعه کیفی
چکیده فارسی مقاله زمینه و هدف: در سال 1374 دستورالعمل اداره نظام نوین امور بیمارستان ها(کارانه) ابلاغ گردید. مطابق این طرح درصدی از درآمدهای اختصاصی بیمارستان تحت عنوان کارانه به پرسنل آن اختصاص می یافت. این طرح به مدت 20 سال در بیمارستانها اجرا شد. در اوایل مهر 1393 دستورالعمل جدید طرح کارانه با عنوان طرح مبتنی بر عملکرد با هدف رفع نواقص طرح قبلی مورد بازبینی قرار گرفت و به اجرا درآمد. مواد و روش ها: تحقیق حاضر به شیوه کیفی بود. از مصاحبه عمیق برای گردآوری داده ها استفاده شد. نمونه های مورد بررسی یازده نفر از مدیران میانی و ارشد بیمارستانها و ستاد مرکزی دانشگاه علوم پزشکی اصفهان بودندکه به روش هدفمند انتخاب شدندروش تحلیل داده ها در این پژوهش بر مبنای روش تحلیل موضوعی بود. یافته ها: در این پژوهش، هشت تم اصلی به دست آمدکه عبارتند از: سیاست گذاری صحیح، اصلاح سیستم مدیریت در نظام سلامت، بسترسازی فرهنگی در حمایت از بحث نظارت، برنامه ریزی صحیح، ایجاد انگیزه در پزشکان، اصلاح زیرساختها، بهبود سیستم آموزش و اطلاع رسانی و ایجاد تعاملات سازنده مدیران ارشد و مسئولان مراکز. نتیجه گیری: بارزترین یافته های این پژوهش شامل سیاست گذاری صحیح، بسترسازی فرهنگی در حمایت از بحث نظارت و ایجاد تعاملات سازنده مدیران ارشد و مسئولان مراکز بود.این یافته ها به سیاستگذاران بخش سلامت کمک می کند تا بتوانند با درنظر گرفتن راهکارهای اصلاحی، راهبردهای مناسبی در جهت اجرای نظارت و بهبود آن در طرح کارانه جدید طراحی نمایند. واژه های کلیدی: بیمارستان، کارانه، نظارت

عنوان انگلیسی Reform strategies to improve the performance of surveillance and improve the infrastructure of the implementation of the new scheme of fee guidelines, 2014: A qualitative study
چکیده انگلیسی مقاله Abstract Introduction: In year 1995 new system hospitals administration plan (fee for service), was notified. Under the scheme percentage of specific revenues as a fee for the hospital staff allocated. This scheme was implemented in hospitals for 20 years. In the early September 2104 new guidelines as design performance-based fee for service plan with the aim deficiencies in the previous plan was reviewed and implemented in hospitals in Isfahan. Methods: This study was a qualitative study. In-depth interviews were used to collect data. Eleven of the top and middle managers of hospitals and the headquarters of Isfahan and some faculty members of Isfahan University and hospital officials frequently were chosen Targeted by method. Data analysis in this research was based on thematic analysis. Result: In this study, eight main themes Strategies for structural reforms promote Performance Monitoring and improve infrastructure, implementation of the new guidelines is the fee for service plan Obtained Which include: The correct policy, Reform the system of management in the health system, Cultural context in support of monitoring, to plan Correct, Motivate doctors and structural reforms, improve the education system and to inform and engage senior managers and directors of centers manufacturer. Conclusion: The most notable findings include the correct policy, cultural context and engage constructively in support of senior managers and officials monitoring centers. These findings will help policy makers to consider the proposed revisions with appropriate strategies to monitor and improve the implementation of the new fee plan will be designed.
