بررسی تاثیر پماد موضعی تهیه شده از گیاه فرفیون (Euphorbia macroclada Boiss) بر وضعیت علائم بالینی بیماران استئوآرتریت اولیه زانو: یک مطالعه کارآزمایی بالینی تصادفی دو سو کور |

سابقه و هدف: استئوآرتریت شایعترین علت درد زانو است و روشهای درمانی مختلفی دارد. گیاه فرفیون یک گیاه دارویی مهم در طب سنتی است که در سراسر جهان مورد استفاده قرار میگیرد .این مطالعه با هدف بررسی تأثیر پماد گیاهی فرفیون بر علائم بالینی بیماران مبتلا به استئوآرتریت اولیه زانو، انجام پذیرفت.
مواد و روش ها: این کارآزمایی بالینی تصادفی دوسوکور، بر روی ۵۰ بیمار مبتلا به استئوآرتریت اولیه زانو در کلینیک باغبان درمانگاه روماتولوژی مازندران انجام شد .بیماران به دو گروه ۲۵ نفره تقسیم شدند و فرفیون یا دارونما دریافت کردند. گروه اول تحت درمان دارویی+ پماد موضعی فرفیون و گروه دوم تحت درمان دارویی+ پلاسبو قرار گرفتند. درمان دارویی شامل کپسول سلکسیب 100 میلیگرم و قرص امگا 3 یک عدد روزانه بود. علائم بیماران در ابتدای مطالعه، دو هفته و چهار هفته پس از درمان بررسی شد و نتایج مطالعه با پرسشنامههای WOMAC و KOOS ارزیابی شد. دادهها با استفاده از نرمافزار SPSS 19و با سطح معنیداری 0/05< P تجزیه و تحلیل شدند.
یافتهها: در شروع مطالعه، هیچ تفاوتی بین دو گروه از نظر سن، قد، وزن، جنسیت، شاخص توده بدنی، مزاج و زانوی درگیر، وجود نداشت. طبق مقیاس KOOS، بیماران تحت درمان با فرفیون در مقایسه با دارونما، بهبود بیشتری در علائم بالینی، خشکی، درد، عملکرد روزانه، ورزش و کیفیت زندگی نشان دادند .این بهبود فقط در شاخص ورزش از نظر آماری معنیدار بود (0/036= P). مقیاس WOMACدر پایان مطالعه هیچ تفاوت آماری معنیداری بین گروههای دارونما و فرفیون نشان نداد.
استنتاج: مصرف پماد فرفیون به مدت 4 هفته، میتواند بهطور قابل توجهی وضعیت ورزش کردن بیماران مبتلا به استئوآرتریت اولیه زانو را بهبود بخشد.

Investigating the Effect of a Topical Ointment Prepared from the Ferfion Plant (Euphorbia macroclada Boiss.) on Clinical Symptoms in Patients with Primary Knee Osteoarthritis: A Double-Blind Randomized Clinical Trial |

Background and purpose: Osteoarthritis is the most common cause of knee pain and has various treatment modalities. The Ferfion plant is a significant medicinal plant in traditional medicine used worldwide. This study aimed to investigate the effect of Ferfion herbal ointment on the clinical symptoms of patients with primary knee osteoarthritis.
Materials and methods: This double-blind, randomized clinical trial was conducted on 50 patients with primary knee osteoarthritis at Baghban Clinic. Patients were divided into two groups of 25, receiving either Ferfion ointment or a placebo. The first group received standard drug treatment plus topical Ferfion ointment, and the second group received standard drug treatment plus a placebo ointment. Drug treatment included celecoxib 100 mg capsules and one omega-3 tablet daily. Patients' symptoms were assessed at baseline, two weeks, and four weeks after treatment initiation. Study outcomes were evaluated using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and the Knee injury and Osteoarthritis Outcome Score (KOOS) questionnaires. Data were analyzed using SPSS version 19, with a significance level of P < 0.05.
Results: At the beginning of the study, there were no significant differences between the two groups in age, height, weight, gender, body mass index, temperament, or side of the affected knee. According to the KOOS scale, patients treated with Ferfion showed greater improvement in clinical symptoms, joint stiffness, pain, activities of daily living, sports and recreational activities, and quality of life compared to the placebo group. However, this improvement was statistically significant only in the sports and recreational activities subscale (p=0.036). The WOMAC scale showed no statistically significant difference between the placebo and Ferfion groups at the end of the study.
Conclusion: The use of Ferfion ointment for four weeks significantly improved sports and recreational function in patients with primary knee osteoarthritis.
