چکیده فارسی مقاله Normal 0 false false false MicrosoftInternetExplorer4 /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal"; mso-tstyle-rowband-size:0; mso-tstyle-colband-size:0; mso-style-noshow:yes; mso-style-parent:""; mso-padding-alt:0in 5.4pt 0in 5.4pt; mso-para-margin:0in; mso-para-margin-bottom:.0001pt; mso-pagination:widow-orphan; font-size:10.0pt; font-family:"Times New Roman"; mso-ansi-language:#0400; mso-fareast-language:#0400; mso-bidi-language:#0400;} مقدمه: افزایش هزینه‌های بیمارستانی در سال‌های اخیر و نیز اجرای طرح خودگردانی و اداره‌ی بیمارستان‌ها به وسیله‌ی درآمد اختصاصی آنها، بیمارستان‌ها را با مشکلات مالی جدی رو به رو ساخته است. در این راستا، پژوهشگران بر آن شدند تا به منظور کاهش مقدار کسورات حاصل از عدم ثبت اقدامات انجام گرفته برای بیماران در صورتحساب هزینه‌ی آنان، تأثیر مداخله در این روند با استفاده از الگوی 9 مرحله‌ای ارتقای کیفیت فرآیند را بر افزایش درآمد بیمارستان بررسی نمایند. روش بررسی: در این مطالعه‌ی تحلیلی و مداخله‌ای، 350 پرونده از پرونده‌های بیماران بستری در بخش‌های داخلی مردان و زنان بیمارستان نمازی شیراز در نیمه‌ی دوم سال 1384، به صورت تصادفی ساده انتخاب و با استفاده از فرم‌های جمع‌آوری داده، بررسی و با صورتحساب‌های مربوط مقایسه شد؛ سپس با استفاده از اطلاعات حاصله، فرآیند ثبت اقدامات دارای تعرفه با استفاده از الگوی 9 مرحله‌ای ارتقای کیفیت فرآیند، از طریق تهیه‌ی فرم اقدامات درمانی، آموزش و ارائه‌ی مشاوره به کارکنان در رابطه با نحوه‌ی تکمیل فرم و اهمیت این کار، به مدت 4 ماه مورد مداخله قرار گرفت. در مرحله پس از مداخله، تأثیر مداخله بر روی 215 پرونده در طول 4 ماهه‌ی اول سال 1385 مورد بررسی قرار گرفت و یافته ‌ها با نرم‌افزار SPSS تحلیل شدند. یافته‌ها: قبل از مداخله، 56 درصد از هزینه‌ی اقدامات درمانی انجام گرفته برای بیمار ثبت نشده بود که بعد از مداخله این درآمد از دست رفته به 31 درصد کاهش یافت و در مجموع باعث افزایش 25 درصدی درآمد این بیمارستان در بخش‌های مورد مطالعه شد. نتیجه‌گیری: با صدور صورتحساب هزینه‌ی بیمار مطابق با اقدامات ثبت شده در پرونده‌ی پزشکی وی و در نتیجه اعمال مدیریت مالی صحیح و اجرای الگوی ارتقای کیفیت در این زمینه، بیمارستان می‌تواند درآمد خود را به مقدار قابل توجهی افزایش دهد. واژه‌های کلیدی: درآمد؛ بیمارستان‌ها؛ مدارک پزشکی.

On the other hand, many resources in hospitals become useless because of the wrong and inappropriate financial management. The aim of this study was decreasing the deficits resulting from not recording these procedures in patients' medical records and also in patients' bills using a 9 step quality- improved module on hospital revenues. Methods: This wsa an interventional and longitudinal study on 350 patients' medical records in male and female internal ward in Nemazi hospital in Shiraz in early months of 2006. The randomly selected records were compared with patients' bills. Then a medical procedure form was designed and the nurses were instructed on how to fill this form and were made aware about the importance of this form. The intervention last about 4 months. After intervention in post test the impact of intervention on 215 medical records was examined. Results: In the pretes,t 56% of the costs of medical procedures have not been recorded and after intervention these lost revenues decreased to 31% and totally caused 25% increase in the revenues of this hospital. Conclusion: Nemazi hospital does not have any acceptable method for recording the medical procedures. This hospital can significantly increase its revenues, if patients' bills be compromised with their medical processes records and a correct and appropriate financial management module correctly be used.
