چکیده فارسی مقاله Normal 0 false false false MicrosoftInternetExplorer4 /* Style Definitions */ table.MsoNormalTable {mso-style-name:"Table Normal"; mso-tstyle-rowband-size:0; mso-tstyle-colband-size:0; mso-style-noshow:yes; mso-style-parent:""; mso-padding-alt:0in 5.4pt 0in 5.4pt; mso-para-margin:0in; mso-para-margin-bottom:.0001pt; mso-pagination:widow-orphan; font-size:10.0pt; font-family:"Times New Roman"; mso-ansi-language:#0400; mso-fareast-language:#0400; mso-bidi-language:#0400;} مقدمه: کمیته‌ی مدارک پزشکی سیاست‌ها، روش‌ها، فرم‌ها و کلیه‌ی مسایل مربوط به مدیریت بخش مدارک پزشکی و اطلاعات بیماران را بررسی می‌کند. در خصوص اهمیت آن کمیته، می‌توان به الزامات موجود در دستورالعمل ارزشیابی و اعتبار بخشی بیمارستان‌ها اشاره نمود. این مطالعه با هدف بررسی نحوه‌ی عملکرد کمیته‌ی مدارک پزشکی بیمارستان‌های آموزشی و غیر آموزشی سراسر استان مازندران بر مبنای صورت‌جلسات ارسال شده به اداره کل مدارک پزشکی، مستقر در حوزه‌ی معاونت درمان دانشگاه، انجام شد. روش بررسی: این مطالعه توصیفی- مقطعی، به روش سرشماری 257 صورت‌جلسه‌ی ارسالی از سال 1384 لغایت نیمه‌ی اول 1386 بررسی گردید. ابزار گردآوری داده، چک لیستی بود که عناصر اطلاعاتی آن به استناد دستورالعمل ارزشیابی بیمارستان، مصوب وزارت بهداشت، طراحی شد. متغیرهای مورد بررسی شامل تعداد جلسات تشکیل شده، مسایل مطروحه، ترکیب جلسه و تصمیمات متخذه بود. داده‌ها با استفاده از نرم‌افزار SPSS آنالیز شد. یافته‌ها: از تعداد 600 جلسه که باید در کل دو سال و نیم، در بیمارستان‌ها تشکیل می‌شد، 257 جلسه تشکیل شده بود. کل موارد مطروحه در ارتباط با 5 وظیفه‌ی تعریف شده برای کمیته‌ی مدارک پزشکی، 513 مورد بود که برای 2/70 درصد تصمیم‌گیری شده، برای 8/9 درصد تصمیم‌گیری نشده ، 1/19 درصد نیز غیر قابل حل و 9/0 درصد نامشخص باقی مانده است. نتیجه‌گیری: عملکرد کمیته‌ی مدارک پزشکی بیمارستان‌های استان از نظر تشکیل جلسات، پی‌گیری مصوبات و حضور اعضاء چندان مطلوب نیست. به منظور بهبود وضعیت، برگزاری مرتب جلسات، پیگیری مصوبات و همچنین ارزیابی عملکرد کمیته به صورت سالانه توسط بیمارستان توصیه می‌گردد. واژه‌های کلیدی: مدارک پزشکی؛ کمیته‌های کادر حرفه‌ای؛ مدیریت اطلاعات؛ بیمارستان‌ها؛ کنترل فرم‌ها و مدارک

This committee makes decisions on medical records policy, medical records procedures, medical records forms, and relevant problems to the management of patient information. The medial records committee is important in accreditation programs too. This research was done in respect of assess the performance of medical records committees, according to medical records sessions reports. Methods: In this cross sectional study, all reports of medical records committee during 2005-2007 were observed. The check list for gathering data was designed accordaning to the medical records committee duty in the text books. Variables were the number of medical records committee sessions, discussed problems, made decisions, and the representative which must be present in sessions. Results: Of the 20 hospitals, 5 (30.8%) were educational. The whole sessions which must be established were 600, but only 257 (42.8 %) were established. The discussed problems in relation to 5 responsibility of medical records committee were 513 topics which 360 of them (70.2%) were agreed, 50 (9.8%) refused, and 98 problem (19.1%) were questionable. As the results showed, the physician and financial representative were absent in 63 medical records committee sessions (36.2%). Conclusion: Medical records committee must be more active than the past. Because of importance of medical record committee, the sessions must be establish regularly, the decisions mus follow up, and evaluation of the medical records committee should be done by own hospital annually.
