عنوان فارسی توسعه یک مدل دو مرحله‌ای احتمالی برای مکان‌یابی سلسله‌مراتبی مراکز درمانی بیماری های قلبی با در نظر گرفتن نرخ خدمت‌دهی (یک مطالعه موردی در مرکز قلب تهران)
چکیده فارسی مقاله زمینه و هدف:مکان‌یابی مراکز درمانی از جمله مسائل مهمی است که باید از ابعاد متفاوت و با هدف ارتقای خدمت‌دهی به آن توجه شود. در این مقاله، مسئله مکان‌یابی حداکثر پوشش سلسله مراتبی مراکز درمانی بیماری‌های قلبی با دو سطح خدمت‌دهی شامل کلینیک و بیمارستان و با در نظر گرفتن نرخ خدمت‌دهی به بیماران قلبی بررسی شد. هدف اولیه حداقل نمودن نقاط تقاضای پوشش نیافته، و هدف ثانویه کمینه‌سازی نرخ تقاضاهای از دست‌رفته به عنوان معیاری برای حفظ بیماران بالقوه در شعاع پوشش بود. روش: به منظور تحلیل سیستم های صف، سیستم نوبت‌دهی مرکز قلب تهران بررسی شد.روش پیشنهادی یک مدل برنامه‌ریزی ریاضی دومرحله‌ای احتمالی ‌است . به منظور ارزیابی مدل پیشنهادی چند مثال عددی در نرم افزار GAMSاجرا شد. نتایج: نتایج نشان داد بهترین حالت برای احداث مراکز درمانی، احداث کلینیک در مجاورت یک بیمارستان است.اتخاذ تصمیم در ارتباط با مکان یابی مراکز درمانی،با در نظر گرفتن نرخ خدمت‌دهی به بیماران در هر یک از مراکز درمانی موجب افزایش اعتماد به نتیجه حاصل از مدل بهینه‌سازی و کاهش هزینه‌ها و نرخ تقاضای از دست‌رفته شده است. نتیجه‌گیری:مدل پیشنهادی قادر است با الگو قرار دادن مراکز درمانی در مناطق با طیف جمعیتی مشابه، به تصمیم‌گیرندگان برای استقرار بهینه مراکز درمانی در مناطق محروم یاری رساند.مکان یابی کلینیک‌ها و بیمارستان ها با در نظر گرفتن حالت سلسله مراتبی و نرخ خدمت دهی در آنها، موجب افزایش رفاه بیماران خواهد شد.

عنوان انگلیسی Developing a Probabilistic Two-Stage Model for Hierarchical Healthcare Facility Location by Considering the Service Rate (A Case Study of Tehran Heart Center)
چکیده انگلیسی مقاله Background: medical centers location is one of the most important problems, which should be considered in different dimensions to improve the services. In this paper, we consider the hierarchical maximum covering problem for bi-level healthcare systems including Clinics and hospitals, by taking the service rates into account. In this problem, the initial objective is minimizing the uncovered demand nodes, and secondary objective is the minimizing the lost demand rate as a measure of potentially patients' retention in coverage radius. Methods: In this paper, the serving system in the Tehran heart center hospital is considered for queue analyses. The proposed method is a mathematical optimization model called probabilistic two-stage programming model. To evaluate this model, a number of numerical problems are solved through GAMS software. Results: according to the results, we have found that the best condition for locating the medical centers is that the clinic are located adjacent to a hospital. Decision making about the location problem by considering the service rate in each healthcare centers, increases the reliability to the result of the optimization model. Conclusion: The proposed model is able to help the decision makers to attain an optimized decision for locating the medical centers in deprived areas, inspired by the medical centers in areas with similar population. Therefore, if the clinics and hospitals are located by considering the hierarchical mode and their service rates, the patients' welfare will be enhanced.
